摘要 目的探讨多模态导航辅助电生理技术应用于脑胶质瘤治疗的疗效及复发的影响因素。方法收集264例脑胶质瘤患者临床资料进行回顾性分析,患者均接受多模态导航辅助电生理技术治疗,统计疗效、复发情况,Logistic回归方程分析术后复发影响因素。结果264例脑胶质瘤患者定位准确率为100%(264/264),手术时间为(233.42±19.68)min,住院时间(9.73±2.38)d,治疗总有效率为92.80%。术前、出院时KPS评分分别为(65.51±2.43)分、(70.18±3.26)分,差异有统计学意义(P<0.05)。并发症发生率为4.55%;术后6个月复发率为24.89%(61/245)。Logistic回归方程分析显示,非全切、高级别、癫痫发作、肿瘤直径>6 cm、IDH1突变型是脑胶质瘤术后复发危险因素,术后放化疗、术前KPS评分>70分是脑胶质瘤术后复发保护因素(P<0.05)。结论多模态导航辅助电生理技术用于治疗脑胶质瘤的效果显著,但受多种因素影响,术后存在复发风险,临床工作中需对合并危险因素患者,加强防治,以降低术后复发率,改善预后。 Objective To investigate the effect of multimodal navigation-assisted electrophysiological technology on the curative effect and recurrence of glioma,and use Logistic regression equation to analyze the influencing factors of postoperative recurrence.Methods The clinical data of 264 patients with glioma were collected for retrospective analysis,all of which underwent multi-modal navigation assisted electrophysiological technology.The curative effect and recurrence were counted,and the logistic regression equation was used to analyze the influencing factors of postoperative recurrence.Results 264 patients with glioma were treated with multi-modal navigation assisted physiological technology.The positioning accuracy rate was 100%(264/264),and the operation time was(233.42±19.68)min,the length of hospital stay was(9.73±2.38)days,and the total effective rate of treatment was 92.80%.The KPS scores before operation and at discharge were(65.51±2.43)points and(70.18±3.26)points respectively,and the difference between the two was statistically significant(P<0.05).The complication rate was 4.55%;the recurrence rate at 6 months after operation was 24.89%(61/245).Logistic regression analysis showed that non-total resection,high-grade,epileptic seizures,tumor diameter>6 cm,and IDH1 mutant type were risk factors for postoperative recurrence of glioma.Postoperative radiotherapy and chemotherapy,preoperative KPS score>70 points were the protective factors for postoperative recurrence of glioma(P<0.05).Conclusion The multimodal navigation-assisted electrophysiological technique has a significant effect in the treatment of glioma,but it is affected by many factors and there is a risk of recurrence after surgery.In clinical work,it is necessary to strengthen prevention and treatment of patients with combined risk factors to reduce postoperative recurrence rate and improve prognosis.
