参芪健脾合剂治疗儿童脾虚肝旺型厌食症的效果及对患儿血清Leptin、GAS、SS水平的影响

Efficacy of Shenqi Jianpi Mixture in the treatment of children with anorexia of spleen-deficiency and liver-hyperactivity type and its effect on children's serum Leptin,gastrin,and somatostatin level

目的观察参芪健脾合剂治疗儿童脾虚肝旺型厌食症的效果,并探讨其对患儿血清瘦素(Leptin)、胃泌素(GAS)、生长抑素(SS)水平的影响。方法选取上海市浦东新区中医医院于2019年2月至2021年2月收治的88例脾虚肝旺型厌食症患儿进行前瞻性研究,按照随机数表法分为观察组和对照组各44例。对照组患儿给予葡萄糖酸锌治疗,观察组患儿给予参芪健脾合剂治疗。治疗1个疗程(14 d)后比较两组患儿的临床治疗效果,以及治疗前后的中医证候积分、血清Leptin、GAS、SS水平、体质量和体质量指数(BMI)。结果治疗后,观察组患者的治疗总有效率为93.18%,明显高于对照组的75.00%,差异具有统计学意义(P<0.05);治疗后,观察组患儿的神疲、面色萎黄、食量减少等中医证候积分分别为(0.56±0.06)分、(0.57±0.06)分、(0.62±0.07)分,明显低于对照组的(1.41±0.14)分、(1.34±0.14)分、(1.59±0.17)分,差异均具有统计学意义(P<0.05);治疗后,观察组患儿的血清Leptin、GAS水平分别为(1.59±0.51)μg/L、(188.58±18.53)pg/mL,明显高于对照组的(1.22±0.37)μg/L、(176.56±18.29)pg/mL,观察组患儿血清SS水平为(18.66±2.97)pg/mL,明显低于对照组的(20.11±2.35)pg/mL,差异均具有统计学意义(P<0.05);治疗后,观察组患儿体质量、BMI分别为(21.02±2.29)kg、(16.32±1.80)kg/m^(2),明显高于对照组的(19.01±2.15)kg、(14.02±1.55)kg/m^(2),差异均具有统计学意义(P<0.05)。结论参芪健脾合剂治疗儿童脾虚肝旺型厌食症能有效降低患儿的SS水平,提高Leptin、GAS水平,促进患儿营养吸收,临床应用效果显著。

Objective To study the effect of Shenqi Jianpi Mixture in treating children with anorexia of spleen-deficiency and liver-hyperactivity type,and to explore its effect on the levels of serum leptin(Leptin),gastrin(GAS),and somatostatin(SS)in children.Methods A prospective study was conducted on 88 children with anorexia of spleen-deficiency and liver-hyperactivity type who were admitted to the Traditional Chinese Medicine Hospital of Shanghai Pudong New District from February 2019 to February 2021.According to the random number table method,the children were divided into an observation group and a control group,with 44 cases in each group.The control group was treated with zinc gluconate,and the observation group was treated with Shenqi Jianpi Mixture.After a course of treatment(14 days),the clinical treatment effects of the two groups of children,as well as the TCM syndrome scores,serum Leptin,GAS,SS levels,body mass and body mass index(BMI)before and after treatment were compared.Results After treatment,the total effective rate of the observation group was 93.18%,which were significantly higher than 75.00%in the control group(P<0.05).After treatment,the scores of TCM syndromes such as fatigue,chlorosis,and decreased appetite in the observation group were(0.56±0.06)points,(0.57±0.06)points,and(0.62±0.07)points,which were significantly lower than(1.41±0.14)points,(1.34±0.14)points,and(1.59±0.17)points in the control group(P<0.05).After treatment,the levels of serum Leptin and GAS in the observation group were(1.59±0.51)μg/L and(188.58±18.53)pg/mL,which were significantly higher than(1.22±0.37)μg/L and(176.56±18.29)pg/mL in the control group;the serum SS level of the observation group was(18.66±2.97)pg/mL,which was significantly lower than(20.11±2.35)pg/mL in the control group;the differences were statistically significant(P<0.05).After treatment,the body mass and BMI of the observation group were(21.02±2.29)kg and(16.32±1.80)kg/m^(2),which were significantly higher than(19.01±2.15)kg and(14.02±1.55)kg/m^(2) in the control group(P<0.05).Conclusion Shenqi Jianpi Mixture can effectively reduce children's SS levels,increase Leptin and GAS levels,promote nutrition absorption in children with anorexia of spleen-deficiency and liver-hyperactivity type,and the clinical application effect is significant.
