摘要
目的研究线粒体DNA拷贝数在脑性瘫痪儿童中的表达水平,探讨其在儿童脑性瘫痪中的临床价值。方法2020年1月1日—2020年12月31日在右江民族医学院附属医院儿童保健康复科确诊的50例脑性瘫痪儿童(脑瘫病例组)运用微液滴数字PCR(Droplet Digital PCR,ddPCR)技术进行线粒体DNA拷贝数水平的检测,选取100例健康体检儿童作为健康对照组进行线粒体DNA拷贝数水平的检测。采用Logistic回归分析法和受试者工作特征曲线(ROC)分析线粒体DNA拷贝数与脑瘫发生的相关性和诊断价值。结果无论是从性别、民族还是年龄方面对线粒体DNA拷贝数进行统计学分析,脑瘫病例组的线粒体DNA拷贝数都高于健康对照组,差异有统计学意义(P<0.05),但同组不同性别、不同民族或不同年龄之间的线粒体DNA拷贝数比较,差异没有统计学意义(P>0.05)。不同脑瘫类型间的线粒体DNA拷贝数比较,差异没有统计学意义(P>0.05)。Logistic回归分析在校正了脑性瘫痪相关危险因素后,结果显示线粒体DNA拷贝数表达水平与脑性瘫痪相关联(P<0.05)。ROC曲线显示线粒体DNA拷贝数诊断脑瘫的ROC曲线下面积为0.834,最佳诊断截断值为94.620个/细胞。线粒体DNA拷贝数的诊断效能的灵敏度、特异度分别为79.00%、78.00%。结论脑瘫儿童的线粒体DNA拷贝数高于正常儿童,线粒体DNA拷贝数在儿童脑性瘫痪中具有一定的诊断价值,为儿童脑性瘫痪提供新的早期诊断的指标。
Objective To investigate the expression of mitochondrial DNA copy number in children with cerebral palsy,so as to explore its clinical value in children with cerebral palsy.Methods This study selected a total of 50children with cerebral palsy(cerebral palsy group)diagnosed in the Department of Child Health Care and Rehabilitation in The Affiliated Hospital of Youjiang Medical University for Nationalities from January 1,2020to December 31,2020.Droplet Digital PCR(ddPCR)was employed to detect their mitochondrial DNA copy numbers.At the same time,100healthy children were selected as healthy control group and their mitochondrial DNA copy numbers were detected by ddPCR.Logistic regression analysis and receiver operating characteristic curve(ROC)were adopted to analyze the correlation between mitochondrial DNA copy number and cerebral palsy as well as the diagnostic value of mitochondrial DNA copy number in cerebral palsy.Results According to the statistical analysis of mitochondrial DNA copy number in terms of gender,ethnic groups and age,the mitochondrial DNA copy number of the cerebral palsy group was higher than that of the healthy control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in mitochondrial DNA copy number among different genders,ethnic groups or ages in the same group(P>0.05).There was no statistically significant difference in mitochondrial DNA copy number among different types of cerebral palsy(P>0.05).After adjusting risk factors related to cerebral palsy,Logistic regression analysis showed that the expression of mitochondrial DNA copy number was associated with cerebral palsy(P<0.05).ROC curve showed that the area under ROC curve for the diagnosis of cerebral palsy by mitochondrial DNA copy number was 0.834,and the optimal diagnostic cut-off value was 94.620/cell.The sensitivity and specificity of mitochondrial DNA copy number were 79.00%and 78.00%,respectively.Conclusion The mitochondrial DNA copy number of children with cerebral palsy is higher than that of normal children.Mitochondrial DNA copy number has certain diagnostic value in children with cerebral palsy and provides a new index for early diagnosis of children with cerebral palsy.
