PHQ-9量表在草酸艾司西酞普兰治疗功能性胃肠病患者中的应用研究

Application of PHQ-9 scale in the evaluation of functional gastrointestinal diseases treated with escitalopram oxalate

目的探讨中文版病人健康问卷(PHQ-9)在FGIDs患者中的适用性及评价草酸艾司西酞普兰临床疗效。方法按罗马Ⅳ标准纳入50例功能性消化不良(FD)、肠易激综合征(IBS)伴抑郁症状且经常规治疗无效患者,加用草酸艾司西酞普兰10mg,每天1次,连续服用6个月,治疗前和治疗后4周、12周、24周PHQ-9、GSRS评分变化。结果50例FGIDs患者经草酸艾司西酞普兰治疗后4周PHQ-9评分显著降低,24周达到最低水平,治疗4周胃肠道症状显著改善,并维持至治疗24周。治疗前及治疗后4周、12周、24周PHQ-9评分、GSRS评分差异均有统计学意义(P<0.001)。结论PHQ-9量表简单易行,可有效地筛查和评估患者抑郁情况及评价抗抑郁治疗疗效;艾司西酞普兰不仅有效降低患者抑郁情绪严重程度,且胃肠道症状改善效果显著,疗效持久。

Objective To investigate the applicability of Chinese version of Patient Health Questionnaire(PHQ-9)in patients with functional gastrointestinal diseases(FGIDs)and to evaluate the clinical efficacy of escitalopram oxalate.Methods According to RomanⅣcriteria,this study enrolled 50patients who had FGIDs,irritable bowel syndrome(IBS)complicated with depressive symptoms and failed to respond to conventional treatment.These patients were additionally given 10mg of escitalopram oxalate once daily for 6months.The PHQ-9and GSRS scores were compared before treatment and at 4,12and 24weeks after treatment.Results The PHQ-9scores of these 50patients with FGIDs significantly decreased at 4weeks after treatment with escitalopram oxalate,and reached the lowest level at 24weeks.Their gastrointestinal symptoms improved significantly after 4weeks of treatment and remained unchanged for 24weeks of treatment.There were statistically significant differences in the PHQ-9scores as well as the GSRS scores before treatment and at 4,12and 24weeks after treatment(P<0.001).Conclusion PHQ-9scale is simple and feasible,which can effectively screen and evaluate the depression of patients,as well as the efficacy of antidepressant treatment.The treatment of escitalopram oxalate can not only effectively reduced the severity of depression,but also significantly improved gastrointestinal symptoms with long-lasting curative efficacy.
