健脾化痰方治疗多囊卵巢综合征患者子宫动脉血流的超声指标观察

Ultrasonic index observation on uterine artery blood flow in patients with polycystic ovary syndrome treated by Jianpi Huatan Decoction

目的观察健脾化痰方治疗多囊卵巢综合征(POS)患者子宫动脉血流的超声指标。方法将我院收治的118例POS患者随机分为观察组与对照组各59例。观察组采用健脾化痰方+二甲双胍治疗,对照组采用二甲双胍治疗,比较两组临床疗效,治疗前后子宫内膜厚度和血流,卵巢形态、性激素含量和胰岛素抵抗指数。结果治疗后,观察组总有效率显著高于对照组(P<0.05);两组子宫内膜厚度均增加,血流搏动指数(PI)、血流阻力指数(RI)均下降,且观察组子宫内膜厚度高于对照组,PI、RI指数均低于对照组(P<0.05);两组卵巢体积、卵泡数、卵巢表面积、基质面积均下降,且观察组卵巢体积、卵泡数、卵巢表面积、基质面积均低于对照组(P<0.05);两组雌激素(E2)升高,卵泡生成素(FSH)、促黄体生成素(LH)、睾酮(T)、胰岛素抵抗指数(HOMA-IR)均下降,且观察组E2高于对照组,FSH、LH、T、HOMA-IR均低于对照组(P<0.05)。结论健脾化痰方联合二甲双胍治疗POS患者临床疗效佳,能显著改善子宫动脉血流,促进排卵,调节内分泌功能,改善胰岛功能。彩色多普勒超声在应用于POS患者疗效评价中,对子宫动脉血流变化灵敏,准确评估卵泡发育。

Objective To observe the ultrasound index of uterine artery blood flow in patients with polycystic ovary syndrome(POS)treated by Jianpi Huatan Decoction.Methods A total of 118 POS patients were randomly divided into observation group and control group,59 cases each.Observation group was treated with Jianpi Huatan Decoction and metformin,and control group was treated with metformin.The clinical effects,endometrial thickness and blood flow,ovarian morphology,sex hormone content and insulin resistance index before and after treatment of the two groups were compared.Results After treatment,the total effective rate of observation group was significantly higher than that of control group(P<0.05);the endometrial thickness of both groups increased;the blood flow pulsation index and blood flow resistance index of both groups decreased,and were lower in observation group than control group(P<0.05);ovarian volume,number of follicles,ovarian surface area,stromal area of both groups decreased,and were lower in observation groups than control group(P<0.05);estrogen(E2)increased,FSH,LH,testosterone(T),insulin resistance index(HOMA-IR)decreased in both groups,and E2 was higher,FSH,LH,T,and HOMA-IR were lower in observation group than control group(P<0.05).Conclusion Jianpi Huatan Decoction combined with metformin has a good clinical effect in treatment of POS and can significantly improve uterine artery blood flow,promote ovulation,regulate endocrine function,and improve pancreatic islet function.Color Doppler ultrasound is sensitive to changes in uterine artery blood flow in the evaluation of the efficacy of POS,and accurately assesses follicular development.
