手术联合心理干预对老年股骨粗隆间骨折患者的效果

Effect of surgical combined with psychological treatment in elderly patients with femoral intertrochanteric fracture

目的探讨手术联合心理干预对老年股骨粗隆间骨折(Intertrochanteric fracture of femur,IFF)患者的效果及对其情绪、心理的影响。方法选定本院2019年1月~2021年1月住院治疗的120例老年股骨粗隆间骨折患者,采用随机数字表法分为参照组60例、实验组60例,参照组患者采纳保守治疗及常规干预,实验组患者采纳股骨近端防旋髓内钉(Proximal femur anti-rotation intramedullary nail,PFNA)术联合心理干预,比较两组髋关节功能优良率,比较对比不同年龄段、不同病情阶段、不同居住地、不同文化程度患者焦虑自评量表(Self-Rating Anxiety Scale,SAS)、抑郁自评量表(Self-rating depression scale,SDS)、匹兹堡睡眠质量指数(Pittsburgh Sleep Quality Index,PSQI)评分。结果实验组髋关节功能优良率(96.67%)高于参照组(76.67%),实验组治疗后不同年龄段、不同疾病分型、不同居住地、不同文化程度患者SAS评分、SDS评分均比参照组低,实验组PSQI评分低于参照组(P<0.05)。结论PFNA手术联合心理干预可有效改善老年IFF患者髋关节功能,减轻消极情绪,改善睡眠质量,效果显著。

Objective To analyze the effect of surgical combined with psychological treatment in elderly patients with femoral intertrochanteric fracture.Methods 120 elderly patients with femoral intertrochanteric fracture hospitalized in our hospital from January 2019 to January 2021 were randomly divided into 60 cases in the reference group and 60 cases in the experimental group.The patients in the reference group were treated with conservative treatment and routine intervention,The patients in the experimental group were treated with proximal femoral anti rotation intramedullary nail(PFNA)combined with psychological intervention.The excellent and good rate of hip joint function was compared between the two groups,and the self rating Anxiety Scale(SAS)was compared between patients of different ages,different disease stages,different places of residence and different educational levels Self rating Depression Scale(SDS)and Pittsburgh sleep quality index(PSQI).Results The excellent and good rate of hip joint function in the experimental group(96.67%)was higher than that in the reference group(76.67%).After treatment,the SAS score and SDS score of patients of different ages,different disease types,different places of residence and different educational levels in the experimental group were lower than those in the reference group.The PSQI score of the experimental group was lower than that of the reference group(P<0.05).Conclusion PFNA surgery combined with psychological intervention can effectively improve hip function,reduce negative emotion and improve sleep quality in elderly patients with femoral intertrochanteric fracture.
