股前外侧嵌合皮瓣与串联皮瓣修复口腔颌面部肿瘤根治术后缺损的修复效果分析

Effect analysis of anterolateral femoral chimeric flap and tandem flap for repairing defects after radical resection of oral and maxillofacial tumors

目的分析股前外侧嵌合皮瓣与串联皮瓣两种修复方法对口腔颌面部肿瘤根治术后缺损的作用。方法回顾性分析院内2017年1月-2020年1月73例口腔颌面部肿瘤术后缺损患者,其中观察组(股前外侧嵌合皮瓣修复)34例,对照组(串联皮瓣修复)39例,对比两组的修复效果。结果与对照组比较,观察组总手术时间、后首次经口进食时间、皮瓣愈合时间均较短,但皮瓣制作+吻合时间较长,术后3月,术后12月,观察组外形、咀嚼功能、闭合情况、语言功能、容水情况、饮食吞咽6项评分均较高,术后6月,观察组总并发症率11.76%(4/34)低于对照组35.90%(14/39),差异具有统计学意义(P<0.05)。结论口腔颌面部肿瘤术后缺损以股前外侧嵌合皮瓣修复效果更加理想,术后恢复速度较快,安全性好,配合义齿修复,能保障患者的咀嚼效率与牙周健康,具有推广价值。

Objective To analyze the effects of anterolateral femoral chimeric skin flap and tandem skin flap on the defect of oral and maxillofacial tumor after radical resection.Methods From January 2017 to January 2020,73 patients with oral and maxillofacial tumor defects after surgery in our hospital were retrospectively analyzed,including 34 cases in the observation group(anterolateral thigh flap repair)and 39 cases in the control group(serial flap repair),and the repair effects of the two groups were compared.Results Compared with the control group and observation group total operation time,for the first time through the mouth after eating time and skin flap healing time is shorter,but the flap+match for a long time,after 3 months,in December,after observing group appearance,chewing function,closed situation and language function,let the water situation,diet swallowing six grades are high,after June,The total complication rate of the observation group(11.76%,4/34)was lower than that of the control group(35.90%,14/39),and the difference was statistically significant(P<0.05).Conclusion Compared with serial flap repair,anterolateral femoral chimeric flap repair for patients with oral and maxillofacial tumor defect after surgery has an ideal effect,with faster postoperative recovery and better safety.Combined with denture repair,it can guarantee the masticatory efficiency and periodontal health of patients,which has the value of popularization.
