红细胞参数及计算公式在小细胞贫血中的鉴别价值 被引量:2

Value of erythrocyte parameters and calculation formula in differential diagnosing small cell anemia

摘要 目的探讨红细胞参数及计算公式(MCI公式)在鉴别小细胞贫血中的价值。方法收集本院2019年1月—2021年4月的地中海贫血(thalassemia)与缺铁性贫血(iron definiency anemia,IDA)作为研究对象,评估2组红细胞参数及不同计算公式的差异,评估MCI公式的诊断效能。结果地中海贫血组红细胞参数RBC、HGB、HCT、MCHC均明显高于IDA组(P<0.05);地中海贫血组MCV、RDW均明显低于IDA组(P<0.05);2组MCH差异无统计学意义(P>0.05)。地中海贫血与IDA组不同参数计算公式比较,除SI公式外,其他不同参数计算公式差异均有统计学意义(P<0.05)。其中MCI公式AUC最大(AUC=0.890),为诊断效能最高;按照约登指数最大法(YI=0.607)设置诊断截断点,MCI公式敏感度为83.3%,特异度为77.4%。结论在基层医院,红细胞参数及计算公式(MCI公式)在鉴别小细胞贫血中具有较大的筛查价值。 Objective To explore the value of erythrocyte parameters and calculation formula(MCI formula)in differential diagnosing small cell anemia.Methods Thalassemia and iron deficiency anemia(IDA)cases in our hospital from January2019 to April 2021 were collected as the research objects.The differences of erythrocyte parameters and different calculation formulas between the two groups were evaluated,and the diagnostic efficacy of MCI formula was evaluated.Results RBC,HGB,HCT and MCHC in thalassemia group were significantly higher than those in IDA group(P<0.05).The parameters of MCV and RDW in thalassemia group were significantly lower than those in IDA group(P<0.05).There was no statistical significance on the difference in MCH parameters between the two groups(P>0.05).There was statistical significance on the differences in different parameter calculation formulas between thalassemia and IDA group except SI formula(P<0.05).The area under the curve of MCI formula was the largest(AUC=0.890),which was the highest diagnostic efficiency.The cut-off point was set according to the maximum method of Youden’s index(YI)(YI=0.607).The sensitivity of MCI formula was(83.3%)and the specificity was 77.4%.Conclusion In grass-roots hospitals,erythrocyte parameters and calculation formula(MCI formula)has great value in the screening of small cell anemia.
