外固定架三维结构固定儿童股骨远端A型骨折 被引量:5

摘要 目的探讨应用外固定架三维结构固定儿童股骨远端A型骨折的疗效。方法2016年9月~2020年12月,对13例儿童股骨远端A型骨折应用外固定架三维结构固定。年龄7~13岁,平均10.6岁。体重36~59 kg,平均47.7 kg。均为闭合伤。AO分型A1型9例(除外A1.1型),A2型2例,A3型2例。受伤至手术时间2~6 d,平均3.5 d。Kolmert标准评定疗效。结果术后均有钉眼周围红肿及少量渗出,无钉道感染。术后3个月骨折均愈合良好并拆除外固定架。按Kolmert标准,优12例,良1例。术后随访8个月~4年,平均15.5月,无再骨折。结论对儿童股骨远端A型骨折,应用外固定架进行简单且稳定的结构固定,操作简单,疗效满意。 Objective To explore the clinical effect of three-dimensional fixation with external fixator for type A fractures of distal femur in children.Methods From September 2016 to December 2020,13 children were treated with three-dimensional fixation with external fixator for type A fractures of distal femur.The age ranged 7-13 years old,with an average of 10.6 years old.The body weight was 36-59 kg,with an average of 47.7 kg.All were closed injuries.According to the AO classification there were 9 cases of type A1(excluding type A1.1),2 cases of type A2 and 2 cases of type A3.The time from injury to operation was 2-6 d(mean,3.5 d).The curative effect was evaluated by the Kolmert standard after operation.Results After the operation,all the patients had redness,swelling and a small amount of exudation around the needle site,and there was no needle site infection.The fractures healed well and the external fixator was removed after 3 months.According to the Kolmert standard,12 cases were excellent and 1 case was good.All the patients were followed up for 8 months to 4 years,with an average of 15.5 months.No re-fracture occurred.Conclusion For type A fracture of distal femur in children,external fixator technology is a simple and stable structural fixation with good effect.
