腹腔镜卵巢囊肿核除术、阴式卵巢囊肿核除术治疗卵巢成熟畸胎瘤的疗效对比

Comparison of Therapeutic Effects of Laparoscopic Ovarian Cystectomy and Vaginal Cystectomy on Ovarian Teratoma

目的:探讨腹腔镜卵巢囊肿核切除与阴道卵巢囊肿核切除治疗成熟卵巢畸胎瘤的应用效果。方法:在本院2018年9月~2020年2月诊治的卵巢成熟畸胎瘤患者中选出178例为研究对象,根据患者的手术治疗意愿分组:腹腔镜组和阴式组,收集两组患者的术后恢复指标等。结果:阴式组患者的手术时间和术后住院时间均短于腹腔镜组,且术中出血量较腹腔镜组患者更少,P均<0.001;在术后首次排气时间、术后镇痛药物使用率和术后并发症发生率对比上,组间差异无统计学意义,P>0.05;两组患者术后3个月的卵巢功能指标(LH、FSH、P、E2)均较术前有明显改善,P<0.001;组间卵巢功能指标对比术前术后均无统计学意义,P>0.05。结论:腹腔镜手术和阴式手术在卵巢成熟畸胎瘤治疗中均有显著疗效,但阴式手术更有利于缩短手术时间,减少术中出血,促进患者术后康复。

Objective:To study the effect of laparoscopic ovarian cystectomy and negative ovarian resection in the treatment of ovarian teratoma.Methods:178 patients with ovarian teratoma were selected from September 2018 to February 2020.According to the patients’intention,they were divided into laparoscopic group and negative group.The postoperative recovery indicators of the two groups of patients were collected.Results:The operation time and hospital stay were shorter than those in laparoscopy group,and the bleeding was less than that in laparoscopy group(P<0.001).There was no significant difference in the first exhaust time,the use rate of postoperative analgesics and the incidence of postoperative complications between the two groups(P>0.05);The ovarian function indexes(LH,FSH,P,E2)of the two groups at 3 months after operation were significantly improved compared with those before operation(P<0.001);There was no significant difference in ovarian function between groups before and after operation(P>0.05).Conclusion:Laparoscopic surgery and vaginal surgery have significant effect in the treatment of ovarian teratoma,but vaginal surgery is more conducive to shorten the operation time,reduce intraoperative bleeding,and promote postoperative rehabilitation of patients.
