多谱勒超声对脐带囊性病变的诊断价值

Diagnostic Value of Doppler Ultrasound in Cystic Lesions of Umbilical Cord

目的:探讨彩色多谱勒超声对脐带囊性病变的诊断价值。方法:回顾性分析本院2018年10月~2020年10月经超声检查诊断的94例胎儿脐带囊肿孕妇临床资料,所有孕妇产前采用LOGIQ 5 Pro及LOGIQ P9型彩色多普勒超声诊断仪进行检查,记录所有胎儿脐带囊肿的超声检查结果及表现。结果:94例脐带囊肿包括单纯性脐带囊肿72例(单发囊肿58例,多发囊肿14例),囊肿合并其他异常22例;囊肿最大约102mm×97mm×68mm,最小约9mm×7mm×3mm;单纯性脐带囊肿中,位于脐带近胎盘区54例,脐带近胎儿区10例,游离段8例,合并其他异常的脐带囊肿中,位于脐带近胎盘区2例,脐带近胎儿区9例,游离段11例。单纯性脐带囊肿及合并其他异常的脐带囊肿位置占比比较差异有统计学意义(P<0.05)。脐带囊肿合并其他异常胎儿类型主要以心脏及血管发育异常(36.36%)、骨骼系统及肢体发育异常(31.82%)、神经系统发育异常(27.27%)占比较高,单脐动脉(18.18%)、羊水量增多(18.18%)、前腹壁畸形(13.64%)、胎儿发育受限(13.64%)、泌尿系统发育异常(9.09%)、消化系统发育异常(4.55%)。结论:产前彩色多谱勒超声对脐带囊性病变具有重要的诊断价值,通过产前超声检查,有利于脐带囊性病变胎儿的早期诊断及预后预判。

Objective:To investigate the value of color Doppler ultrasound in the diagnosis of cord cystic lesions.Methods:The clinical data of 94 pregnant women with fetal umbilical cord cyst diagnosed by ultrasound in our hospital from October 2018 to October 2020 were retrospectively analyzed.All pregnant women were examined by LOGIQ 5 Pro and LOGIQ P9 color Doppler ultrasound diagnostic instrument before delivery,and the ultrasound results and manifestations of all fetal umbilical cord cyst were recorded.Results:The 94 cases of umbilical cyst included 72 cases of simple umbilical cyst(58 cases of single cyst,14 cases of multiple cyst)and 22 cases of cyst with other abnormalities.The maximum cyst was 102mm×97mm×68mm,and the minimum was 9mm×7mm×3mm.Among the umbilical cord cysts,54 cases were located near the placenta,10 cases were near the fetus and 8 cases were free.Among the umbilical cord cysts with other abnormalities,2 cases were located near the placenta,9 cases were near the fetus and 11 cases were free.There was significant difference in the proportion of umbilical cord cyst location between simple umbilical cord cyst and other abnormalities(P<0.05).Umbilical cord cyst with other abnormal fetal types were mainly heart and vascular abnormalities(36.36%),skeletal system and limb abnormalities(31.82%),nervous system abnormalities(27.27%).Single umbilical artery(18.18%),increased amniotic fluid(18.18%),anterior abdominal wall malformation(13.64%),fetal restriction(13.64%),abnormal development of urinary system(9.09%),abnormal development of digestive system(4.55%).Conclusion:Prenatal color Doppler ultrasonography is of great value in the diagnosis of umbilical cord cystic lesions.Prenatal ultrasonography is beneficial to the early diagnosis and prognosis of fetuses with cord cystic lesions.
