GE Revolution 256排螺旋CT动态增强及多模式后处理技术在结肠癌诊断中的应用

Application of GE Revolution 256-slice spiral CT dynamic enhancement and multi-mode post-processing technology in the diagnosis of colon cancer

目的探讨应用GE Revolution 256排螺旋CT动态增强及多模式后处理技术在结肠癌诊断中的应用价值。方法抽取2020年1月至2021年6月在新乡市中心医院行手术治疗的112例结肠癌患者,所有患者术前均采用GE Revolution 256排螺旋CT进行平扫、动态增强扫描机多模式图像后处理检查,以手术病理结果为金标准,分析GE Revolution 256排螺旋CT诊断结肠癌T分期及N分期的准确率及诊断一致性,分析CT扫描征象与手术及病理结果的一致性。结果GE Revolution 256排螺旋CT扫描结肠癌病变位置、组织学分型及病变形态均与病理检查结果一致。以术后病理检查T分期为金标准,GE Revolution 256排螺旋CT扫描诊断结肠癌准确率为90.18%(101/112),Kappa=0.805,P<0.001,一致性好。以术后病理检查N分期为金标准,GE Revolution 256排螺旋CT扫描诊断结肠癌准确率为88.39%(99/112),Kappa=0.772,P<0.001,一致性好;GE Revolution 256排螺旋CT扫描结果与手术及病理检查结果在肿块边缘不规则、肠壁增厚、肠腔狭窄、病灶累及浆膜层、病灶累及邻近组织器官、肠梗阻、腹膜后淋巴结转移及肠系膜血管受累等方面均具有良好的一致性(Kappa值=0.522、0.739、0.944、0.820、0.834、0.772、0.602、0.875,P均<0.001)。结论结肠癌患者术前采用GE Revolution 256排螺旋CT动态增强及多模式后处理技术进行临床诊断及术前分期结果与手术病理结果一致性良好,诊断准确率高,是术前评估病情、选择治疗方案的重要参考依据。

Objective To investigate the clinical value of GE Revolution 256-slice spiral CT dynamic enhancement and multi-modal post-processing technology in the diagnosis of colon cancer.Methods A total of 112 patients with colon cancer who were treated by surgery in Xinxiang Central Hospital from January 2020 to June 2021 were selected.All patients were taken GE Revolution 256-slice spiral CT for plain scan and dynamic enhancement scanner multi-mode image post-processing examination before operation.Taking the pathological results as the gold standard,the accuracy and diagnostic consistency of GE Revolution 256-slice spiral CT in the diagnosis of colon cancer T staging and N staging were analyzed,and the consistency of CT scan signs with surgical and pathological results was analyzed.Results GE Revolution 256-slice spiral CT scan were consistent with the pathological examination results in screening colon cancer lesion location,histological classification and lesion morphology.With the T stage of postoperative pathological examination as the gold standard,the accuracy of GE Revolution 256 slice spiral CT in the diagnosis of colon cancer was 90.18%(101/112),Kappa=0.805,P<0.001,suggesting a good consistency.With postoperative pathological examination N staging as the gold standard,the accuracy of GE Revolution 256-slice spiral CT in the diagnosis of colon cancer was 88.39%(99/112),Kappa=0.772,P<0.001,suggesting a good consistency.The results of GE Revolution 256-slice spiral CT scanning were in good consistency with the results of operation and pathological examination in terms of irregular edge of mass,thickening of intestinal wall,stenosis of intestinal cavity,lesion involving serosa,lesion involving adjacent tissues and organs,intestinal obstruction,retroperitoneal lymph node metastasis and mesenteric vascular involvement(Kappa=0.522,0.739,0.944,0.820,0.834,0.772,0.602,0.875;all P<0.001).Conclusions GE Revolution 256-slice spiral CT dynamic enhancement and multi-mode post-processing technology,Preoperative in clinical diagnosis and preoperative staging of colon cancer patients,are in good consistency with surgical and pathological results,and have high diagnostic accuracy.It was an important reference basis for preoperative evaluation of the condition and selection of treatment plan.
