精神心理因素对结肠镜肠道准备质量的影响

The influence of psychological factors on the quality of bowel preparation for colo-noscopy

目的初步研究精神心理因素对结肠镜肠道准备质量的影响。方法选取2021年4月至2021年9月在我院拟行结肠镜检查的患者,收集所有符合标准患者的一般资料,采用广泛性焦虑量表(Generalized Anxiety Disorder 7-item,GAD-7)、患者健康问卷抑郁量表(Patient Health Questionnaire-9,PHQ-9)评估患者精神心理状态,采用波士顿肠道准备评价量表(Boston bowel preparation scale,BBPS)评估肠道清洁程度,分析焦虑、抑郁对肠道准备的影响。结果316例接受结肠镜检查患者符合标准其中焦虑、抑郁、焦虑合并抑郁的发生率分别为27.5%、35.4%和9.8%。肠道准备不合格组的焦虑、抑郁、焦虑合并抑郁的发生率均高于合格组,其中肠道准备合格组和不合格组的抑郁发生率分别为22.4%和34.1%,差异有统计学意义(P<0.05)。单纯抑郁组肠道准备不合格率为37.0%,高于对照组肠道准备不合格率21.6%,差异有统计学意义(P<0.05)。单纯抑郁组右侧结肠、中段结肠BBPS评分较对照组降低,差异有统计学意义(P<0.05),对照组、单纯抑郁组、焦虑合并抑郁组的左侧结肠BBPS评分均低于单纯焦虑组,差异有统计学意义(P<0.05)。结论精神心理因素是结肠镜肠道准备的影响因素,其中抑郁降低右侧结肠及中段结肠清洁程度最为明显。

Objective To preliminary study the influence of psychological factors on the quality of bowel preparation for colonoscopy.Methods The general data of all patients were collected who scheduled to undergo colonoscopy in Tangdu Hospital from Apr.2021 to Sep.2021.The anxiety and depression of all patients were surveyed using Generalized Anxiety Disorder 7-item(GAD-7)and Patient Health Questionnaire-9(PHQ-9).The quality of bowel preparation was analyzed using Boston bowel preparation scale(BBPS).The influence of anxiety and depression on bowel preparation was analyzed.Results The incidences of anxiety,depression,anxiety combined with depression in 316 patients undergoing colonoscopy were 27.5%,35.4%,and 9.8%,respectively.The incidence of anxiety,depression,and anxiety combined with depression in the unqualified bowel preparation group was significantly higher than that in the qualified group(P<0.05).The incidences of depression in the qualified and unqualified group were 22.3%and 35.4%respectively,and the difference was statistically significant(P<0.05).The unqualified rate of bowel preparation in the depression group(37.0%)was higher than that in the control group(21.6%),the difference was statistically significant(P<0.05).The BBPS scores of the right colon and transverse colon in the depression group were lower than those in the control group,the difference was statistically significant(P<0.05).The BBPS scores of the left colon in the control group,depression group,anxiety combined with depression group were lower than those in anxiety group,the difference was statistically significant(P<0.05).Conclusion Psychological factors are the influencing factors of bowel preparation for colonoscopy,and depression reduces the cleanliness of the right and transverse colons most obviously.
