国家早期预警评分在心脏外科潜在危重症患者病情早期识别中的应用

The application of national early warning scoring system in the early identification of patients with cardiac surgery

目的基于国家早期预警评分系统制定心脏外科潜在危重症患者评估流程,探讨其在潜在危重症患者早期识别中的应用效果。方法选取宁夏医科大学总医院心脏中心外科病区2019年6—12月114例住院患者为对照组,2020年1—6月117例住院患者为干预组,对照组采用科室常规病情识别方案,干预组采用医护一体化的预警评分流程进行病情识别。比较2组潜在危重症患者早期识别率、病情好转、转入CICU及意外死亡发生情况。结果干预组潜在危重症患者早期识别率和病情好转人数较对照组显著提高,且转入CICU比例低于对照组,差异均有统计学意义(P<0.05)。结论通过实施医护一体化路径式早期预警工作流程,可显著提高病区医护人员对潜在危重症患者的早期识别率,减少非计划性CICU转入率,进而可提升医疗护理质量,保障患者安全。

Objective Based on the national early warning scoring system to develop the assessment procedures and to explore its application effect in early identification for potentially critical patient in cardiac surgery.Methods We recruited 114 hospitality patients with cardiovascular disease from June to December 2019 as the control group,and 117 during same year from January to July 2020 as the experimental group.The assessment procedures of the national early warning scoring was used in the experimental group to assess the condition of potentially critical patient,and the control group received conventional observation.Duration of critical period,the early identification rate,improvement of the disease and re-occupancy rate of CICU were compared between the two groups.Results The early recognition rate of potential illness patients in the intervention group was significantly improved,and the condition outcome was also better than that of the control group with statistically significant difference(P<0.05).Conclusion The implementation of integrated medical and medical early warning workflow can significantly improve the early recognition rate of medical staff to potentially critically ill patients,reduce the unplanned CICU transfer rate of patients,and thus improve the quality of medical care and ensure the safety of patients.
