舒筋活血汤联合雷火灸治疗踝关节撞击综合征的临床疗效观察

Clinical Study of Shujin Huoxue Decoction Combined with Thunder-Fire Moxibustion on Ankle Impingement Syndrome

目的:观察舒筋活血汤联合雷火灸治疗踝关节撞击综合征的临床疗效。方法:选择2020年10月—2021年5月在黑龙江中医药大学附属第一医院门诊就诊的患者74例,随机分为对照组与试验组各37例。对照组给予口服舒筋活血汤及功能训练的传统治疗,试验组在对照组治疗的基础上增加雷火灸治疗。比较两组治疗前后的效果;血清TNF-α水平;治疗前、治疗2周、治疗1个月的Baird-Jackson评分和AOFAS踝与后足功能评分。结果:治疗1个月后试验组总有效率高于对照组(P<0.05);治疗前两组TNF-α水平比较,差异无统计学意义(P>0.05),治疗1个月后试验组的TNF-α水平低于对照组(P<0.01);治疗前两组Baird-Jackson评分和AOFAS踝与后足功能评分比较,差异无统计学意义(P>0.05),治疗2周及治疗1个月的Baird-Jackson评分和AOFAS踝与后足功能评分比较,试验组的评分均高于对照组评分(P<0.01)。结论:舒筋活血汤联合雷火灸治疗踝关节撞击综合征有优良的治疗效果,较传统治疗有见效快、复发概率小的优点,更好、更快地使患者恢复踝关节功能,减轻疼痛。

Objective: To observe the clinical efficacy of Shujin Huoxue Decoction combined with thunder-fire moxibustion in the treatment of ankle impingement syndrome. Methods: A total of 74 patients were randomly divided into the control group and the study group, with 37 patients in each group. The control group was treated with Shujin Huoxue Decoction and functional training;on which basis, the study group was also treated with thunder-fire moxibustion. The clinical efficacy was compared between the two groups. The serum level of TNF-α, Baird-Jackson score and AOFAS ankle and hind foot function score were assessed before the treatment, after two weeks of the treatment and after one month of the treatment in the two groups. Results: After one month of the treatment, the total effective rate was higher in the study group than that in the control group(P<0.05). There was no statistical difference in TNF-α level between the two groups before the treatment(P>0.05);the level of TNF-α was lower in the study group than that in the control group after one month of the treatment(P<0.01). There were no statistical differences in Baird-Jackson score and AOFAS ankle and hind foot function score between the two groups before the treatment(P>0.05), Baird-Jackson score and AOFAS ankle and hind foot function score were higher in the study group than those in the control group after two weeks of treatment and after one month of treatment(P<0.01). Conclusion: Shujin Huoxue Decoction combined with thunder-fire moxibustion has excellent therapeutic effect on ankle impingement syndrome. Compared with traditional treatment, Shujin Huoxue Decoction has the advantages of quicker effect and less recurrence probability, which can better and faster restore ankle function and relieve pain.
