目前,与生活方式及行为有关的慢性非传染性疾病已成为影响人类健康的社会公共问题,对于其管理需综合协调,形成预防、诊治、医养、康复一体化的管理思维,即以全科医学为基础的社区作为服务平台,遵循“五大处方”,以心脏康复运动为核心,在安全保障前提下,使慢病患者从医师支配的“治”,过渡到由自我主导的“康养”,从而提高患者依从性、改善患者生活质量、降低再住院率、死亡率及医疗负担。

At present,chronic non communicable diseases related to lifestyle and behavior have become social and public problems affecting human health.Their management needs comprehensive coordination to form a management thinking of the integration of prevention,diagnosis and treatment,medical care and rehabilitation,that is,the community based on general practice medicine is used as the service platform,the"five Prescriptions"are followed,the heart rehabilitation movement is the core,and under the premise of safety and security,make patients with chronic diseases transit from“treatment”dominated by doctors to“health care”dominated by themselves,so as to improve patients'compliance,improve patients'quality of life,and reduce rehospitalization rate,mortality and medical burden.
