胃癌腹膜转移腹腔热灌注化疗药物的研究进展

Research progress of intraperitoneal hyperthermic perfusion chemotherapy drugs for peritoneal metastasis of gastric cancer

腹膜转移是胃癌常见的转移方式之一,腹膜转移往往与生存质量下降、生存期短相关。传统的放化疗等手段未能取得明显疗效。众多研究表明腹腔热灌注化疗(Hyperthermic intraperitoneal chemotherapy,HIPEC)在胃癌腹膜转移治疗中起关键作用,单独HIPEC或联合手术等治疗可以预防腹膜转移的发生,显著延长患者的生存期。但是,关于HIPEC的药物选择没有指南推荐,大多中心按照既往研究和经验选择。目前大多数研究推荐铂类及紫杉烷类药物作为胃癌HIPEC药物。因此,本文就胃癌HIPEC临床研究中药物的选择进行归纳总结,以期为临床医师提供胃癌HIPEC药物选择方案。

Peritoneal metastasis is one of the common ways of metastasis of gastric cancer.It is often associated with de⁃creased quality of life and short survival.Traditional therapies such as radiotherapy and chemotherapy fail to achieve sig⁃nificant efficacy.Studies have shown that HIPEC plays a key role in the treatment of peritoneal metastasis of gastric can⁃cer.Treatment with HIPEC alone or combined with surgery can prevent peritoneal metastasis and significantly prolong their survival time.However,there are no guidelines for HIPEC drug selection,and most centers choose according to previ⁃ous research and experience.At present,most studies recommend platinum and taxanes as HIPEC drugs for gastric can⁃cer.This paper summarizes the drug selection in the clinical study of gastric cancer HIPEC,so as to provide clinicians with the drug selection scheme of gastric cancer HIPEC.
