摘要
目的在进展型(FIGOⅢ/Ⅳ)高级别浆液性卵巢癌(HGSOC)中检测细胞角蛋白19(CK19)mRNA的表达情况和意义。方法在128例进展型HGSOC中应用RT-PCR法检测CK19 mRNA的表达,分析其临床意义。采用MTT实验、transwell小室实验和流式细胞学实验检测CK19对卵巢癌细胞增殖和侵袭能力的影响。结果进展型HGSOC中CK19 mRNA的表达与腹水情况和铂类耐药相关,Kaplan-Meier生存分析显示CK19 mRNA高表达组患者5年无进展生存期明显降低(P=0.015),但总生存期差异无统计学意义(P=0.320)。多因素Cox回归分析显示CK19 mRNA高表达是5年无进展生存期的独立危险因素(P=0.049)。体外实验显示CK19过表达的OVMZ-6细胞增殖能力和侵袭能力明显提高(P<0.05)。结论进展型HGSOC中CK19表达上调可增加细胞的增殖和侵袭能力,影响患者5年无进展生存期,提示CK19可能是进展型HGSOC精准治疗的新靶点。
Objective To investigate the expression and clinical significance of cytokeratin 19(CK19)in advanced high-grade(FIGOⅢ/Ⅳ)serous ovarian cancer(HGSOC).Methods The CK19 mRNA expression was assessed by RT-PCR in 128 cases of advanced HGSOC.The effect of CK19 on cell proliferation and migration were elucidated by MTT assay,trans-well migration assay and flow cytometry in vitro.Results CK19 mRNA level was significantly associated with ascitic fluid and platinum resistance.Kaplan-Meier curve showed that elevated CK19 mRNA expression predicted shorter 5-year progression-free survival(PFS),while there was no significant association between CK19 mRNA and overall survival(OS).Increased CK19 mRNA expression was identified as an significant independent poor prognostic factor for 5-year PFS(P=0.049),but not OS(P=0.320).CK19 promoted proliferation and migration of ovarian cancer cells(P<0.05).Conclusion CK19 overexpression promotes proliferation and migration of ovarian cancer cells,resulting in poor 5-year PFS in patients with advanced HGSOC.CK19 may represent attractive therapeutic target in advanced high-grade serous ovarian cancer.
