自拟消肿方联合冰硝散治疗三踝骨折肿胀的疗效观察

Curative Effect of Self-made Prescription combined with Bingxiao Powder on the Treatment of Swelling of Trimalleolar Fracture

目的探讨应用自拟消肿方内服联合冰硝散外敷对三踝骨折患者肿胀问题的临床治疗效果。方法选取2020年1月-2021年7月在山东中医药大学附属医院骨科入院治疗的三踝骨折患者76例,采用随机数字表法分为对照组(38例)和治疗组(38例)。对照组应用20%甘露醇注射液进行消肿治疗,治疗组在对照组消肿治疗方案的基础上,给予自拟消肿方汤药内服,同时联合冰硝散外敷进行消肿治疗。比较两组用药1 d、3d、5d后的肿胀值、肿胀程度及不良反应情况,并分析其临床疗效。结果治疗1d后,两组肿胀值均较治疗前无明显变化(P>0.05);治疗3d后,两组肿胀值均较治疗前降低(P<0.05),且治疗组显著低于对照组(P<0.01);治疗5d后,两组肿胀值均较治疗前显著降低(P<0.01),且治疗组显著低于对照组(P<0.01)。治疗后,两组肿胀评分均较治疗前显著降低(P<0.01),且治疗组低于对照组(P<0.05)。治疗后,治疗组临床总有效率(94.74%)高于对照组(84.21%,P<0.05)。结论内服自拟消肿方联合外敷冰硝散能快速缓解三踝骨折肿胀情况,有利于缩短手术等待时间,值得临床推广应用。

Objective To investigate the clinical effect of self-made internal prescription combined with external application of Bingxiao powder on the treatment of swelling of trimalleolar fracture.Methods Seventy-six cases of trimalleolar fracture from January 2020 to July 2021 in the Department of Sports Injury Orthopedics of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine were selected,and divided into a treatment group and a control group by the random number table method,with 38 cases in each group.The control group was injected with 20%mannitol to treat swelling.Based on the treatment of the control group,the treatment group was treated with self-made internal prescription combined with external application of Bingxiao powder.The swelling value and degree and adverse reactions of cases in the two groups were recorded after one day,three days,and five days of treatment,and the clinical effect of self-made internal prescription combined with external application of Bingxiao powder was analyzed.Results After one day of treatment,the swelling value of the two groups had no significant change compared with that before treatment(P>0.05).After three days of treatment,the swelling value of the two groups was lower than that before treatment(P<0.05),and the swelling value of the treatment group was significantly lower than that of the control group(P<0.01).After five days of treatment,the swelling value of the two groups was significantly lower than that before treatment(P<0.01),and the swelling value of the treatment group was significantly lower than that of the control group(P<0.01).After treatment,the swelling scores of the two groups were significantly lower than those before treatment(P<0.01),and the swelling score of the treatment group was lower than that of the control group(P<0.05).The total clinical effective rate of the treatment group(94.74%)was higher than that of the control group(84.21%,P<0.05)after treatment.Conclusion Self-made internal prescription combined with external application of Bingxiao powder can quickly relieve the swelling of trimalleolar fracture,which is beneficial to shorten the waiting time for surgery and is worthy of clinical application.
