针灸联合康复训练对脑卒中偏瘫患者下肢功能恢复的干预效果

Intervention Effect of Acupuncture and Moxibustion Combined Rehabilitation Training on Lower Limb Function Recovery in Stroke Patients with Hemiplegia

目的探究脑卒中偏瘫患者采用针灸与康复训练联合治疗方案对下肢功能恢复的影响。方法选择我院2020年7月-2021年7月收治住院的100例脑卒中偏瘫患者作为研究对象。采用随机数字表法对患者分组,对照组50例给予康复训练,试验组50例给予针灸联合康复训练。比较两组患者治疗前后下肢功能恢复效果(FMA评分)、步态分期、日常生活自理能力(ADL评分)。结果治疗前两组患者下肢FMA评分比较差异无统计学意义(P>0.05),治疗后两组患者下肢FMA评分均增加,与治疗前比较,差异有统计学意义(P<0.05)。试验组治疗后下肢FMA评分高于对照组,差异有统计学意义(P<0.05);治疗前两组患者步态分期中患侧、双侧支撑期比例差异无统计学意义(P>0.05),治疗后患者患侧支撑期比例相比于治疗前提升,双侧支撑期比例相比于治疗前下降,差异有统计学意义(P<0.05);试验组患侧支撑期比例高于对照组,双侧支撑期比例低于对照组,差异有统计学意义(P<0.05)。与治疗前比较,患者ADL评分中各项得分均提高,差异有统计学意义(P<0.05);治疗后试验组ADL评分高于对照组,差异有统计学意义(P<0.05)。结论在脑卒中偏瘫患者康复训练基础上,增加针灸治疗可促进患者下肢功能恢复,改善步态,提高生活自理能力,具有较好的临床治疗效果,值得推广。

Objective To explore the effect of acupuncture and moxibustion combined rehabilitation training on lower limb function recovery of stroke patients with hemiplegia.Methods One hundred stroke patients with hemiplegia admitted to our hospital from July 2020 to July 2021 were selected as the study subjects.They were divided into two groups by random number table.Patients in control group(50)received rehabilitation training,and those in experimental group(50)were provided acupuncture and moxibustion combined rehabilitation training.Lower limb function recovery[Fugl-Meyer Assessment(FMA)scale],gait cycle and daily self-care ability[Activities of Daily Living(ADL)score]was compared between the two groups before and after treatment.Results There was no statistically significant difference in the FMA scores of lower limbs between the two groups before treatment(P>0.05),while the FMA score of lower limbs in both groups increased markedly after treatment(P<0.05).The experimental group had higher FMA score of lower limbs than control group(P<0.05).Before treatment,no difference was found in the proportion of the affected side and double support stage in the gait cycle between the two groups(P>0.05).After treatment,the proportion of the affected side support stage increased while the proportion of double support stage decreased compared with that before treatment(P<0.05).The experimental group had higher proportion of the affected side support stage and lower proportion of double support stage than control group(P<0.05).Compared with before treatment,ADL scores of patients were improved(P<0.05).The experimental group outweighed control group in ADL score after treatment(P<0.05).Conclusion On the basis of rehabilitation training for stroke patients with hemiplegia,acupuncture and moxibustion therapy can facilitate the lower limb function recovery and improve gait and self-care ability,which has an excellent clinical treatment effect and is worth promoting.
