摘要
目的分析急性心肌梗死(STEMI)梗死相关动脉(IRA)为第一对角支患者的心电图特点,统计其中出现南非国旗征的概率,为临床医师提供准确快捷的诊断方法。方法回顾分析2016年1月至2020年12月武汉亚洲心脏病医院AMI经冠脉造影确定IRA为第一对角支患者的心电图资料19例,分析患者入院时心电图各导联ST段抬高或压低的情况。结果18例(94.7%)有I和aVL导联ST段抬高,胸前导联ST段抬高依次为:V2导联12例(63.2%)、V3、V4导联各2例(10.5%)、V1、V5、V6导联各1例(5.3%)。19例患者均有III导联ST段压低;其次为aVF导联15例(78.9%)、Ⅱ导联12例(63.2%),aVL导联1例(5.3%)。胸前导联ST段压低的有V3、V4、V5各1例(5.3%)。梗死相关动脉为第一对角支的心电图中,南非国旗征出现概率为63.2%。结论梗死相关动脉为第一对角支患者的心电图中南非国旗征出现概率较高,可为提高诊断正确率提供可靠依据。
Objective To analyze the electrocardiographic(ECG)characteristics for the first diagonal branch of infarction relatedartery(IRA)in patients with acute ST-segment elevation myocardial infarction(STEMI).We observed the incidence rate of South African Flag Sign,in order to help the physician to make a quick diagnosis.Methods A total of 19 STEMI patients with coronary angiography(CAG)confirmed first diagonal branch of IRA were retrospectively analyzed.The patients were treated in our hospital from January 2016 to December 2020 and their ECG changes at admission were studied for ST-segment elevation/depression changes at different leads in all patients.Results Eighteen(94.7%)patients had ST-segment elevation in lead I and aVL.The incidence of ST-segment elevation in the chest lead was,in turn as 12(63.20%)patients in lead V2,2(10.5%)in each lead V3 and lead V4,1(5.3%)in lead V1,lead V5 and lead V6;while ST-segment depression was as 19(100%)patients in lead III,15(78.9%)in lead aVF,12(63.2%)in lead II and 1(5.3%)in lead aVL respectively.The probability of South African Flag Sign was 63.2%.Conclusions The incidence of South African Flag Sign in the first diagonal branch of infarction relatedartery(IRA)in patients with acute ST-segment elevation myocardial infarction(STEMI)was relatively high,which provides a reliable basis for doctors to improve their diagnostic accuracy.
