老年COPD合并肺部感染患者呼出气一氧化氮水平与EOS、PCT、CRP水平相关性分析 被引量:16

摘要 目的分析老年慢性阻塞性肺疾病(COPD)合并肺部感染患者呼出气一氧化氮(FeNO)水平与嗜酸性粒细胞(EOS)、降钙素原(PCT)、C反应蛋白(CRP)水平的相关性。方法选取2019年2月—2021年2月西宁市第一人民医院收治的112例老年COPD患者,统计老年COPD患者肺部感染发生情况,将并发肺部感染患者记为研究组,将未并发肺部感染患者记为对照组。比较两组患者FeNO、EOS、PCT、CRP水平,采用单因素分析两组患者临床特征,采用Logistic回归分析判定影响老年COPD患者并发肺部感染的因素,采用Pearson相关性分析老年COPD合并肺部感染患者FeNO水平与EOS、PCT、CRP水平的关系。结果研究组患者FeNO、EOS、PCT、CRP水平均高于对照组(P<0.05);研究组患者呼吸衰竭构成比及机械通气时间、白细胞计数、中性粒细胞计数、红细胞沉降率(血沉)与对照组患者比较,差异均有统计学意义(P<0.05);Logistic多因素回归分析,结果显示呼吸衰竭、白细胞计数、FeNO、EOS、PCT、CRP均是老年COPD患者并发肺部感染的相关因素(P<0.05);经Pearson相关性检验,老年COPD合并肺部感染患者FeNO水平与PCT、CRP水平均呈正相关(P<0.05),与EOS水平不相关(P>0.05)。结论老年COPD合并肺部感染患者FeNO、EOS、PCT、CRP水平均异常升高,且FeNO水平与PCT、CRP水平均呈正相关。 Objective To analyze the level of exhaled nitric oxide(FeNO)and the levels of eosinophils(EOS),procalcitonin(PCT)and C-reactive protein(CRP)in elderly patients with chronic obstructive pulmonary disease(COPD)and lung infection Correlation.Methods 112 elderly COPD patients admitted to Xining First People's Hospital from February 2019 to February 2021 were selected.The incidence of lung infections in elderly COPD patients was accounted,patients with concurrent lung infections were recorded as the study group,and patients without lung infections were recorded as the control group.The levels of FeNO,EOS,PCT and CRP of the twogroups of patients were compared,the clinical characteristics of the two groups of patients were analyzed by single factor.Logistic regression analysis was used to determine the factors affecting pulmonary infection in elderly COPD patients,Pearson correlation was used to analyze the relationship between FeNO levels and EOS,PCT and CRP levels in elderly patients with COPD and lung infection.Results The levels of FeNO,EOS,PCT and CRP in the study group were higher than those in the control group(P<0.05).Comparing with the control group,the difference in the proportion of respiratory failure and the duration of mechanical ventilation,white blood cell count,neutrophil count,erythrocyte sedimentation rate in the study group was statistically significant(P<0.05).Logistic multivariate regression analysis showed that respiratory failure,white blood cell count,FeNO,EOS,PCT,and CRP were all related factors for pulmonary infection in elderly COPD patients(P<0.05).According to Pearson correlation test,FeNO levels in elderly patients with COPD complicated with pulmonary infection were positively correlated with PCT and CRP levels(P<0.05),it was no correlation with EOS level(P>0.05).Conclusion The levels of FeNO,EOS,PCT,and CRP in elderly patients with COPD and pulmonary infection are abnormally increased,and the levels of FeNO are positively correlated with the levels ofPCT and CRP.
