FOCUS-PDCA管理模式下风险控制干预在肿瘤患者外周静脉置入中心静脉导管护理中的应用效果

Application effect of risk control intervention under FOCUS-PDCA management mode in PICC catheterization nursing of tumor patients

目的 探讨FOCUS-PDCA管理模式下风险控制干预在肿瘤患者外周静脉置入中心静脉导管(PICC)护理中的应用效果。方法 选取2019年1月至2020年12月南平市第一医院收治的首次化疗并行PICC的86例患者作为研究对象,按照收治的时间顺序将2019年1月至12月收治的患者作为对照组(43例),将2020年1月至12月收治的患者作为观察组(43例)。观察组患者采用FOCUS-PDCA管理模式下风险控制干预,对照组患者采用常规护理干预。比较两组的PICC置管时间、非计划拔管率、PICC相关并发症发生情况及护理质量。结果 观察组PICC置管时间长于对照组,差异有统计学意义(P<0.05);观察组非计划拔管率低于对照组,差异有统计学意义(P<0.05);观察组PICC相关并发症总发生率低于对照组,差异有统计学意义(P<0.05);观察组护理质量中置管操作、沟通能力、书写规范、责任心、并发症处置及服务态度评分均高于对照组,差异有统计学意义(P<0.05)。结论 基于FOCUS-PDCA管理模式下风险控制干预应用于肿瘤患者PICC置管护理中,PICC置管时间延长,非计划拔管率降低,相关并发症减少,且可提高护理质量。

Objective To explore the application effect of risk control intervention under FOCUS-PDCA management mode in peripherally inserted central venous catheters(PICC)nursing of tumor patients.Methods A total of 86 patients admitted to the First Hospital of Nanping from January 2019 to December 2020 who received chemotherapy for the first time and PICC were selected as the study subjects.Patients admitted from January to December 2019 were selected as the control group(43 cases)and patients admitted from January to December 2020 were selected as the observation group(43 cases)according to the chronological order of admission.The observation group was treated with FOCUS-PDCA risk control intervention,while the control group was treated with routine nursing intervention.PICC catheterization time,unplanned extubation rate,PICC-related complications and nursing quality were compared between the two groups.Results The catheterization time of PICC in observation group was longer than that in control group,and the difference was statistically significant(P<0.05).The unplanned extubation rate in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).The total incidence of PICC-related complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).The scores of intubation operation,communication ability,writing standard,sense of responsibility,complication treatment and service attitude in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion The risk control intervention based on FOCUS-PDCA management mode applied in the nursing of PICC catheterization in patients with cancer can prolong the catheterization time of PICC,reduce the rate of unplanned extubation,reduce the related complications,and improve the quality of nursing.
