高压氧联合加温湿化高流量鼻导管氧疗治疗中重度阻塞性睡眠呼吸暂停低通气综合征的效果及对呼吸参数、MIP-1α、BNP、HIF-1α的影响 被引量:4

摘要 目的探究高压氧联合加温湿化高流量鼻导管氧疗治疗中重度阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的效果。方法选择2018年9月至2021年10月我院收治的90例中重度OSAHS患者为研究对象,根据治疗方案将其分为对照组与观察组,各45例。对照组接受常规治疗+加温湿化高流量鼻导管氧疗,观察组在对照组基础上给予高压氧治疗。比较两组的治疗效果。结果观察组的治疗总有效率明显高于对照组(P<0.05)。治疗后,两组患者的PaO_(2)、SaO_(2)、RR均高于治疗前,PaCO_(2)、ODI均低于治疗前,且观察组优于对照组(P<0.05)。治疗后,两组患者的MIP-1α、BNP、HIF-1α水平均低于治疗前,且观察组低于对照组(P<0.05)。治疗后,两组患者的AHI均低于治疗前,MSaO_(2)、LSO_(2)均高于治疗前,LAT均短于治疗前,且观察组优于对照组(P<0.05)。治疗后,两组患者的PSQI量表各维度评分均降低,且观察组低于对照组(P<0.05)。结论高压氧联合加温湿化高流量鼻导管氧疗治疗中重度OSAHS的效果满意,不仅能改善患者的呼吸参数,调节睡眠监测参数,改善MIP-1α、BNP、HIF-1α水平,同时还能提高患者睡眠质量。 Objective To explore the effect of hyperbaric oxygen combined with heating and humidifying high-flow nasal catheter oxygen therapy in the treatment of moderate to severe obstructive sleep apnea hypopnea syndrome(OSAHS).Methods A total of 90 patients with moderate to severe OSAHS adimitted in our hospital from September 2018 to October 2021 were selected as the research objects.According to the treatment plans,the patients were divided into control group and observation group,with 45 cases in each group.The control group received conventional treatment+heating and humidification high-flow nasal catheter oxygen therapy,and the observation group received hyperbaric oxygen therapy on the basis of the control group.The treatment effects of the two groups were compared.Results The total effective rate of treatment of the observation group was significantly higher than that of the control group(P<0.05).After treatment,the PaO_(2),SaO_(2) and RR in the two groups were higher than those before treatment,the PaCO_(2) and ODI were lower than those before treatment,and those in the observation group were better than the control group(P<0.05).After treatment,the levels of MIP-1α,BNP and HIF-1α in the two groups were lower than those before treatment,and those in the observation group were lower than the control group(P<0.05).After treatment,the AHI in the two groups was lower than that before treatment,the MSaO_(2) and LSO_(2) were higher than those before treatment,the LAT was shorter than that before treament,and those in the observation group were better than the control group(P<0.05).After treatment,the scores of all dimensions of PSQI scale in the two groups were lower than those before treatment,and those in the observation group were lower than the control group(P<0.05).Conclusion Hyperbaric oxygen combined with heating and humidifying high-flow nasal catheter oxygen therapy in the treatment of moderate to severe OSAHS has satisfactory effect.It can not only improve the patients'respiratory parameters,adjust the sleep monitoring parameters,improve the MIP-1α,BNP and HIF-1αlevels,but also improve the sleep quality of patients.
