无头加压螺钉治疗成人指掌骨骨折的临床效果及2年随访

Clinical Effect and 2-year Follow-up of Headless Compression Screws in the Treatment of Adult Phalangeal Fractures

目的:探讨无头加压螺钉治疗成人指掌骨骨折的临床效果及2年随访情况。方法:收集2018年1月-2020年1月某院接收的50例成人指掌骨骨折患者,根据随机数字表法将其分为对照组和观察组各25例,对照组予以微型钢板治疗,观察组予以无头加压螺钉治疗,两组均实施2年随访,对比两组手术及术后恢复情况、并发症、术后2年手部功能恢复情况。结果:观察组手术用时、术后功能锻炼开始时间、掌指关节活动度恢复时间、骨折愈合时间均比对照组短,且观察组骨折愈合优良率比对照组高(P<0.05);观察组手术并发症发生率是4.00%,比对照组的24.00%低(P<0.05);观察组术后2年中华医学会手部功能评定标准优良率是96.00%,比对照组的72.00%高(P<0.05)。结论:无头加压螺钉治疗成人指掌骨骨折可缩短手术、术后恢复及骨折愈合时间,提升骨折愈合优良率、减少并发症,并且提升远期手功能康复效果。

Objective:To investigate the clinical effect and 2-year follow-up of headless compression screws in the treatment of adult phalangeal fractures.Methods:50 cases of adult patients with phalangeal fractures in a hospital from January 2018 to January 2020 were collected and randomly divided into two groups according to the random number table method.25 cases in the control group were treated with mini-plates,and 25 cases in the observation group were treated with headless compression screws.Both groups were followed up for 2 years,the operation and postoperative recovery,complications and recovery of hand function in the 2 years after the operation were compared between the two groups.Results:The operation time,postoperative functional exercise start time,metacarpophalangeal joint mobility recovery time,and fracture healing time in the observation group were shorter than those in the control group,and the excellent and good rate of fracture healing was higher than that in the control group(P<0.05).The incidence of surgical complications in the observation group was 4.00%,which was lower than 24.00%in the control group(P<0.05).In the observation group,the excellent and good rate of the Chinese Medical Association's hand function evaluation standard at 2 years after operation was 96.00%,which was higher than 72.00%in the control group(P<0.05).Conclusion:The treatment of adult phalangeal fractures with headless compression screws can shorten the operation time,postoperative recovery time and fracture healing time,improve the excellent and good rate of fracture healing,reduce complications,as well as improve the effect of long-term hand function rehabilitation.
