复方丹参滴丸对急性心肌梗死并心衰患者心肌和血管内皮功能的影响

Influence of Compound Danshen Dripping Pills on Myocardial Function and Vascular Endothelial Function in Patients with Acute Myocardial Infarction Complicated with Heart Failure

目的:观察复方丹参滴丸辅助治疗对急性心肌梗死并心衰患者心肌和血管内皮功能的影响。方法:选取某院2018年10月-2019年12月某院住院治疗60例急性心肌梗死并发心衰患者,按随机表法分为观察组和基础组各30例,基础组给予常规治疗,观察组在基础组治疗基础上加用复方丹参滴丸,观察两组疗效、心肌酶谱、心肌功能、血管内皮功能及并发症。结果:治疗后观察组疗效高于基础组,差异有统计学意义(P<0.05);两组患者治疗前左室射血分数(LVEF)及左室舒张末期内径(LVESD)及6min步行试验(6MWT)比较差异无统计学意义(P>0.05),治疗后两组LVEF均有所上升,且观察组高于基础组,LVESD水平均降低,且观察组低于基础组,6MWT距离都有所上升,且观察组高于基础组(P<0.05);治疗后观察组肌酸激酶(CK)峰值、肌酸激酶同工酶(CK-MB)峰值、内皮素-1(ET-1)水平均低于基础组,一氧化氮(NO)水平高于基础组(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:复方丹参滴丸辅助治疗急性心肌梗死并发心衰患者可有效减轻心脏负荷,改善心肌缺血的症状及血管内皮功能,促进心脏功能恢复。

Objective:To observe the influence of compound Danshen dripping pills adjuvant therapy on myocardial function and vascular endothelial function in patients with acute myocardial infarction complicated with heart failure.Methods:A total of 60 patients with acute myocardial infarction complicated with heart failure in a hospital from October 2018 to December 2019 were selected and divided into observation group and basic group according to the random number table method,with 30 cases in each group.Basic group was given conventional treatment,and observation group was given compound Danshen dripping pills on the basis of basic group.The efficacy,myocardial enzymes,myocardial function,vascular endothelial function and complications were observed in the two groups.Results:After treatment,the efficacy of observation group was higher than that of basic group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the left ventricular ejection fraction(LVEF),left ventricular end systolic diameter(LVESD)and 6-min walking test(6MWT)between the two groups before treatment(P>0.05).After treatment,the LVEF of the two groups was increased,the LVEF of observation group was higher than that of basic group,the LVESD level in the two groups was decreased,the LVESD of observation group was lower than that of basic group,the 6MWT distance in the two gorups was lengthened,and the 6MWT distance in observation group was longer than that in basic group(P<0.05).The peak value of creatine kinase(CK),peak value of creatine kinase isoenzyme(CK-MB)and endothelin-1(ET-1)in observation group were lower than those in basic group while the level of nitric oxide(NO)was higher than that in basic group(P<0.05).The difference in the incidence of complications was not significant between the two groups(P>0.05).Conclusion:Compound Danshen dripping pills adjuvant therapy for patients with acute myocardial infarction complicated with heart failure can effectively relieve the heart load,improve the symptoms of myocardial ischemia and vascular endothelial function,and promote the recovery of heart function.
