基层医院数字化X线摄影系统筛查在新型冠状病毒肺炎疫情期间的应用价值

Application Value of Digital Radiography Screening in Primary Hospitals During the COVID-19 Pandemic

目的探讨新型冠状病毒肺炎疫情期间基层医院数字化X线摄影系统(DR)筛查的应用价值。方法回顾性分析2020年1月25日至2月29日仙桃市15个乡镇卫生院的7026例疑似新型冠状病毒肺炎患者的临床资料,所有患者均行DR筛查,以最终临床诊断结果为金标准,统计DR筛查的灵敏度、特异度、准确度,并分析新型冠状病毒肺炎的DR影像学表现。结果最终临床诊断结果显示,7026例疑似患者中,阳性261例,阴性6765例。DR筛查新型冠状病毒肺炎的灵敏度、特异度、准确度分别为89.7%(234/261)、98.7%(6676/6765)、98.3%(6910/7026)。新型冠状病毒肺炎的影像学表现主要包括模糊结节型、模糊斑片状影型、两肺多发片状影中内带为主型、两肺多发片状影外带为主型四种类型。结论基层医院DR筛查在新型冠状病毒肺炎疫情期间发挥着重要作用,其诊断灵敏度、特异度、准确度均较高。

Objective The application value of digital radiography(DR)screening in primary hospitals during the COVID-19 pandemic was explored.Methods The clinical data of 7026 suspected COVID-19 patients in 15 township health centers in Xiantao City from January 25th to February 29th in 2020 was reviewed and all of them have underwent DR inspection.Taken the final clinical diagnostic results as the gold standard,the sensitivity,specificity and accuracy of DR screening were counted and then the DR imaging manifestations of COVID-19 were analyzed.Results The final clinical diagnostic results showed that of all the 7026 suspected COVID-19 patients,261 cases were diagnosed positive,while the other 6765 cases were diagnosed negative.The sensitivity,specificity and accuracy of DR screening applied in COVID-19 were 89.7%(234/261),98.7%(6676/6765)and 98.3%(6910/7026).The imaging manifestations of COVID-19 included 4 types:type of fuzzy nodule,type of fuzzy flake shadow,type of multiple flake shadow mainly in inner(medium part)of both lungs and type of multiple flake shadow mainly in periphery of both lungs.Conclusion DR screening in primary hospitals plays an important role during the COVID-19 pandemic,and its sensitivity,specificity and accuracy are relatively high.
