氨甲环酸辅助PFNA治疗老年股骨转子间骨折效果

Effect of tranexamic acid combined with PFNA in the treatment of elderly intertrochanteric fracture

目的:观察在老年股骨转子间骨折患者PFNA治疗中应用氨甲环酸进行辅助治疗的效果。方法:纳入研究具体时间为2019年4月至2020年4月,通过区组随机化分组法将其中32例患者设为对照组(PFNA治疗);并将另32例患者设为观察组(氨甲环酸辅助PFNA治疗),比较不同组别失血情况及骨折愈合情况。结果:观察组老年股骨转子间骨折患者总失血量相较对照组明显更少(P<0.05);观察组显性失血量相较对照组明显更少(P<0.05);观察组隐性失血量相较对照组明显更少(P<0.05);且观察组住院时长以及骨折愈合时长相较对照组明显更短(P<0.05);观察组血红蛋白浓度相较对照组明显更高(P<0.05);观察组红细胞比容相较对照组明显更高(P<0.05)。结论:氨甲环酸辅助PFNA治疗老年股骨转子间骨折既能够明显改善老年患者围术期失血情况,同时也有助于缩短骨折愈合时间,利于患者恢复,值得应用。

Objective:To observe the effect of tranexamic acid in the treatment of elderly patients with intertrochanteric fracture treated with tranexamic acid.Methods:From April 2019 to April 2020,32 patients were divided into the reference group(PFNA treatment)and the other 32 patients were set as the experimental group(tranexamic acid assisted PFNA treatment),and the blood loss and fracture healing of different groups were compared.Results:The total blood loss of elderly patients with intertrochanteric fracture in the experimental group was significantly lower than that in the reference group(P<0.05);the dominant blood loss in the experimental group was significantly lower than that in the reference group(P<0.05);the hidden blood loss in the experimental group was significantly lower than that in the reference group(P<0.05);the length of hospitalization and fracture healing time in the experimental group was significantly shorter than that in the reference group(P<0.05);the blood loss of the experimental group was significantly lower than that of the reference group(P<0.05).Compared with the control group,the concentration of erythropoietin was significantly increased(P<0.05),and the hematocrit of the experimental group was significantly higher than that of the reference group(P<0.05).Conclusion:Tranexamic acid assisted PFNA in the treatment of elderly intertrochanteric fracture cannot only significantly improve the perioperative blood loss of elderly patients,but also help to shorten the fracture healing time,which is conducive to the recovery of patients,which is worthy of application.
