摘要
围手术期睡眠障碍与生物钟基因密切相关。动物生物钟的循环是一个基因表达的负反馈环路,也是一个基因表达的振荡器,不同麻醉药物暴露会对生物钟基因产生不同的影响。麻醉药物不仅会改变分子生物钟成分表达,还可暂时抑制生物钟基因表达的转录,导致表达周期发生相移;此外,麻醉药物还可通过促进脑和肌肉ARNT样蛋白-1降解发挥作用。全身麻醉对昼夜节律时钟及节律的影响可初步解释全身麻醉对睡眠影响的机制,但麻醉诱导的生物钟分子变化导致行为变化的机制目前尚未明确,仍需未来一系列基础与临床研究探讨。
Perioperative sleep disorders are closely related to clock genes.The cycle of animal biological clock is a negative feedback loop of gene expression,and also an oscillator of gene expression.Exposure to different anesthetics will have different effects on the clock genes.Anesthetics can not only change the expression of molecular clock components,but also temporarily inhibit the transcription of clock gene expression,resulting in phase shift of the expression cycle.In addition,anesthetics can also play a role by promoting the degradation of brain and muscle ARNT like protein-1.These effects of general anesthesia on the circadian clock and its rhythm can initially explain the mechanism of general anesthesia's effect on sleep.However,the mechanism of behavioral changes caused by anesthesia induced circadian clock changes is still unclear,and a series of basic and clinical studies are still needed in the future.
