渐进式直立活动在慢性呼吸衰竭患者无创通气撤机中的应用

Application of gradual upright activities in non-invasive ventilation and weaning in patients with chronic respiratory failure

目的探讨渐进式直立活动在慢性呼吸衰竭患者无创通气撤机中的应用效果。方法采用便利抽样法,选取2018年1月—2020年1月在解放军海军安庆医院接受无创通气治疗的68例慢性呼吸衰竭患者为研究对象,按随机数字表法将患者分为观察组和对照组,各34例。对照组采取常规撤机方法,观察组在对照组基础上开展渐进式直立活动,比较两组患者撤机成功率及并发症发生率。结果观察组患者撤机成功率为85.29%(29/34),高于对照组的61.76%(21/34),差异有统计学意义(P<0.05);观察组患者胃肠胀气、压力性损伤及新发呼吸循环不稳定的发生率均低于对照组,差异均有统计学意义(P<0.05)。结论渐进式直立活动在慢性呼吸衰竭患者无创通气撤机中的应用具有较好效果,能够提高患者的撤机成功率,并降低患者撤机后并发症的发生率,值得推广应用。

Objective To explore the application effect of gradual upright activities in non-invasive ventilation and weaning in patients with chronic respiratory failure.Methods A total of 68 patients with chronic respiratory failure who received non-invasive ventilation in the PLA Navy Anqing Hospital from January 2018 to January 2020 were selected as the research objects by the convenient sampling method.Patients were divided into the observation group and the control group according to the random number table method,with 34 cases in each group.The control group adopted the routine weaning method,while the observation group carried out gradual upright activities on the basis of the control group.The success rate of weaning and the incidence of complications were compared between the two groups.Results The success rate of weaning in the observation group was 85.29%(29/34),which was higher than 61.76%(21/34)in the control group,and the difference was statistically significant(P<0.05).The incidences of flatulence,pressure injury and new respiratory instability in observation group were lower than thoese in control group,and the differences were statistically significant(P<0.05).Conclusions The application of gradual upright activities in non-invasive ventilation and weaning in patients with chronic respiratory failure has a good effect.It can improve the success rate of weaning and reduce the incidence of complications after weaning,which is worthy of promotion and application.
