枸橼酸舒芬太尼注射液联合七氟醚对腹腔镜全子宫切除术患者的影响

Analysis of effect of sufentanil citrate injection combined with sevoflurane on patients with laparoscopic total hysterectomy

目的:分析腹腔镜全子宫切除术中枸橼酸舒芬太尼注射液联合七氟醚麻醉对患者应激反应的影响。方法:回顾性选取2020年2月—2021年5月本院麻醉科腹腔镜全子宫切除术患者90例,依据麻醉方法分为枸橼酸舒芬太尼注射液联合七氟醚麻醉组(七氟醚组)、枸橼酸舒芬太尼注射液联合丙泊酚麻醉组(丙泊酚组)两组,各45例,七氟醚组患者接受枸橼酸舒芬太尼注射液联合七氟醚麻醉,丙泊酚组患者接受枸橼酸舒芬太尼注射液联合丙泊酚麻醉,统计分析两组患者的血流动力学、应激反应、麻醉质量、不良反应发生情况。结果:两组患者T1、T2、T3、T4时的平均动脉压、心率和脑电双频指数之间的差异均不显著(P>0.05)。两组患者T1、T2、T3、T4时的血糖、血管紧张素Ⅱ和皮质醇水平均逐渐升高(P<0.05);T1、T2、T3、T4时七氟醚组患者的血糖、血管紧张素Ⅱ、皮质醇水平均低于丙泊酚组(P<0.05)。七氟醚组患者的自主呼吸恢复时间短于丙泊酚组(P<0.05),睁眼时间短于丙泊酚组(P<0.05),言语应答时间短于丙泊酚组(P<0.05),拔管时间短于丙泊酚组(P<0.05),定向力恢复时间短于丙泊酚组(P<0.05)。七氟醚组患者的不良反应发生率为8.89%(4/45),低于丙泊酚组的28.89%(13/45)(P<0.05)。结论:腹腔镜全子宫切除术中枸橼酸舒芬太尼注射液联合七氟醚较丙泊酚麻醉更能有效减轻患者应激反应。

Objective:To analyze the effect of sufentanil citrate injection combined with sevoflurane anesthesia on stress response in patients undergoing total laparoscopic hysterectomy.Methods:90 cases of patients with total laparoscopic hysterectomy in the Department of Anesthesiology of our hospital from February 2020 to May 2021 were selected retrospectively.According to the anesthesia methods,they were divided into two groups:sufentanil citrate injection combined with sevoflurane anesthesia group(sevoflurane group)and sufentanil citrate injection combined with propofol anesthesia group(propofol group),with 45 cases in each group.Patients in the sevoflurane group received sufentanil citrate injection combined with sevoflurane anesthesia,and patients in the propofol group received sufentanil citrate injection combined with propofol anesthesia.The hemodynamics,stress reactions,anesthesia quality,and occurrences of adverse reactions of the two groups were statistically analyzed.Results:There was no significant difference in mean arterial pressure,heart rate,and bispectral index between the two groups at T1,T2,T3,and T4(P>0.05).The levels of blood glucose,angiotensin Ⅱ,and cortisol at T1,T2,T3,and T4 increased gradually in the two groups(P<0.05).The levels of blood glucose,angiotensin Ⅱ,and cortisol at T1,T2,T3,and T4 in the sevoflurane group were lower than those in the propofol group(P<0.05).The recovery time of spontaneous respiration in the sevoflurane group was shorter than that in the propofol group(P<0.05).The eye-opening time was shorter than that in the propofol group(P<0.05).The speech response time was shorter than that in the propofol group(P<0.05).The extubation time was shorter than that in the propofol group(P<0.05),and the recovery time of directional force was shorter than that in the propofol group(P<0.05).The incidence of adverse reactions in the sevoflurane group 8.89%(4/45)was lower than that in the propofol group 28.89%(13/45)(P<0.05).Conclusion:Sufentanil citrate injection combined with sevoflurane anesthesia in total laparoscopic hysterectomy can more effectively reduce the stress response of patients than propofol anesthesia.
