摘要
目的探讨人表皮生长因子2(HER-2)、血管内皮生长因子(VEGF)及生长抑素受体2(SSTR2)在中老年进展期胃癌组织中的表达及其与胃癌预后的关系。方法选取2014年〜2018年南京大学鼓楼医院收治的接受手术切除的200例中老年进展期胃癌患者作为研究对象,采用免疫组化方法,检测胃癌组织中HER-2、VEGF、SSTR2的表达水平,并结合临床特点对其预后进行多因素分析。结果200例中老年进展期胃癌组织中,HER-2阳性40例(20.00%),VEGF阳性175例(87.50%),SSTR2阳性175例(87.50%)。进展期胃癌患者预后多因素分析结果显示:肿瘤大小、年龄、VEGF为中老年进展期胃癌预后的独立危险因素(P<0.05)。结论中老年进展期胃癌组织中VEGF及SSTR2阳性表达率高,提示其在胃癌的发生、发展过程中存在协同作用;VEGF对中老年进展期胃癌预后具有独立预测意义。
Objective To investigate the expression of human epidermal growth factor 2(HER-2),vascular endothelial growth factor(VEGF)and somatostatin receptor 2(SSTR2)in advanced gastric cancer and their relationship with prognosis in middle-aged and older patients.Methods A total of 200 middle-aged and older patients undergoing surgery for advanced gastric cancer in Nanjing Drum Towel Hospital from 2014 to 2018 were enrolled in this study.Immunohistochemical staining was used to detect the expression of HER-2,VEGF and SSTR2,and their correlations with clinical features and prognosis were analyzed.Results The positive rate of HER-2,VEGF and SSTR2 expression were 20.00%(40 cases),87.50%(175 cases)and 87.50%(175 cases),respectively.Multivariate analysis showed that tumor size,age,VEGF expression were the independent risk factors for prognosis in middle-aged and older patients with advanced gastric cancer(P<0.05).Conclusion High positive rate of VEGF and SSTR2 expression in advanced gastric cancer indicates that VEGF and SSTR2 play an important role in the development and progression of gastric cancer.VEGF expression is an independent predictor for prognosis in advanced gastric cancer in middle-aged and older patients.
