肠道菌群多样性变化与阻塞性睡眠呼吸暂停合并高血压发病的关系研究 被引量:1

摘要 目的探讨肠道菌群多样性变化与阻塞性睡眠呼吸暂停(OSA)合并高血压发病的关系。方法收集2018年7月至2021年6月浙江医院睡眠障碍中心收治的鼾症患者46例,依据多导睡眠图(PSG)监测结果分为单纯鼾症患者(对照组)14例和OSA患者32例,OSA患者根据是否合并高血压再分为OSA合并高血压患者组11例及单纯OSA组21例,3组患者均无重大基础疾病。于PSG监测结束后当日清晨采集粪便样本,并采用16S rRNA测序法作肠道菌群鉴定。采用Metastats分析在不同水平上比较3组患者菌群的差异。结果3组患者中,Firmicutes在OSA合并高血压人群中丰度最高,在对照组中丰度最低;Bacteroidetes在对照组中丰度最高,在OSA合并高血压组中丰度最低;对照组的Bacteroidetes丰度最高,Firmicutes丰度最低;而OSA合并高血压组的Firmicutes丰度最高,Bacteroidetes丰度最低(P<0.05)。结论肠道菌群多样性改变参与OSA合并高血压的发生、发展。 Objective To investigate the relationship between intestinal flora diversity and obstructive sleep apnea(OSA)complicated with hypertension.Methods Forty-six patients with snoring underwent polysomnography(PSG)examination in Sleep Disorders Center of Zhejiang Hospital from July 2018 to June 2021.According to the PSG monitoring results,there were 14 patients with simple snoring(control group)and 32 patients with OSA,among whom 11 cases were complicated with hypertension(OSA and hypertension group)and 21 cases had simple OSA(simple OSA group).None of the three groups had major underlying diseases.Fecal samples were collected and the intestinal flora was identified by 16S rRNA sequencing.Results Among the three groups,Firmicutes had the highest expression in OSA and the lowest expression in the control group,while Bacteroidetes had the highest expression in the control group and the lowest expression in OSA and hypertension group.Metastats analysis showed that the abundance of Bacteroidetes was the highest and Firmicutes was the lowest in the control group,while the abundance of Firmicutes was the highest and Bacteroidetes was the lowest in OSA and hypertension group(P<0.05).Conclusion Changes in intestinal flora diversity may be involved in the occurrence and development of OSA in patients with hypertension.
