某院儿科门诊超说明书用药调查与分析

Investigation and Analysis on Off⁃Label Drug Use in Pediatric Outpatients of a Hospital

目的:调查分析儿科门诊超说明书用药情况,为儿科合理用药提供参考。方法:随机抽取我院2019年7-12月儿科门诊处方,根据药品说明书判断其用药是否超说明书,统计分析超说明书用药类型、患儿年龄段及药品种类分布情况。结果:共抽取门诊处方1200张,含3285条用药医嘱。超说明书用药处方比例为29.42%,超说明书用药医嘱比例为18.60%。超说明用药类型主要是无儿童用药信息(45.17%)和超适应证用药(24.39%)。超说明书用药主要发生在婴幼儿期(32.25%)和学龄前期(21.27%),药物类别以呼吸系统用药(27.17%)、中成药(24.01%)和抗感染药(22.92%)为主。结论:儿科门诊超说明用药较普遍,国家应制定相关法规,医院应加强超说明书用药的监管,以保障儿童用药安全合理。

Objective:To investigate and analyze the off⁃label drug use in the pediatric outpatients,so as to provide reference for rational drug use in pediatrics.Methods:Pediatric outpatient prescriptions in our hospital from Jul.to Dec.2019 were randomly extracted to determine whether off⁃label drug use was used according to drug instructions,and the types of off⁃label drug use,age of children and distribution of drug types were statistically analyzed.Results:A total of 1,200 outpatient prescriptions were selected,including 3,285 medical orders.The proportion of off⁃label drug use prescriptions was 29.42%,and the proportion of off⁃label drug use medical orders was 18.60%.The main types of off⁃label drug use were no information on drug use for children(45.17%)and off⁃label indication(24.39%).Off⁃label drug use mainly occurred in infancy(32.25%)and preschool age(21.27%),and the drug categories were mainly respiratory system drugs(27.17%),Chinese patent medicines(24.01%)and anti⁃infective drugs(22.92%).Conclusion:Off⁃label drug use is common in pediatric outpatients,relevant laws and regulations should be formulated and supervision of off⁃label drug use should be strengthened in hospitals to ensure the safety and rationality of drug use in children.
