文章对从脾论治肝硬化腹水的中西医理论依据和临床运用要点进行了全面探讨,认为西医“肝脏”与中医“脾”具有高度相关性,强调肝硬化腹水的根本病机是脾虚,水湿不运,治疗肝硬化腹水的关键应从脾论治,注重“健脾”能提高肝脏功能,改善肝脏结构。而失代偿期肝硬化的肝脏功能降低和结构改变乃长时间损伤所造成,健脾亦需较长时间才能见到理想效果,故宜“缓图”。临证中注重“健脾”和“缓图”两大基本原则,并贯穿始终,使庞杂理论主次分明,增强了临床疗效,达到了理想的治疗目标。

The article treated from the spleen liver cirrhosis ascites main points of western and Chinese medicine theory and clinical use has carried on the comprehensive discussion.It is concluded that the“liver”of western medicine is highly correlated with the“spleen”of Chinese medicine.It is emphasized that the basic pathogenesis of ascites in cirrhosis is spleen deficiency and water dampness is not transported.The key to the treatment of cirrhotic ascites should be the spleen, focusing on“invigorating the spleen”.Invigorating spleen can improve liver function and improve liver structure.Decompensated cirrhosis results in reduced liver function and structural changes due to long-term injury.Invigorating the spleen also needs a longer time to see the ideal effect, so it is advisable to“slow map”.In clinical diagnosis, the two basic principles of“invigorating the spleen”and“slowing the graph”are emphasized.And throughout, so that the complex theory can make into the primary and secondary clearly.The clinical effect was enhanced, and the ideal therapeutic goal was achieved.
