CGF双膜覆盖应用于上颌前牙区骨缺损引导骨再生术的成骨能力及口腔组织再生效果研究 被引量:7

摘要 目的研究浓缩生长因子(CGF)双膜覆盖应用于上颌前牙区骨缺损引导骨再生术的成骨能力及口腔组织再生效果。方法前瞻性选取2019年1月至2020年10月邯郸市第一医院收治的102例上颌前牙区骨缺损患者作为研究对象。按随机数字表法分为观察组和对照组,每组各51例。观察组实施引导骨再生术(GBR)+Bio-Oss骨粉+CGF膜+海奥生物膜,对照组实施GBR+Bio-Oss骨粉+海奥生物膜。测量患者术后种植体水平向、垂直向骨量以及角化龈宽度数据,采用Jemt牙龈乳头指数(PIS)评价口腔软组织再生效果。结果观察组术后6、12个月种植体水平向、垂直向骨量均显著大于对照组,水平向、垂直向骨吸收量显著低于对照组,差异均有统计学意义(P<0.05)。观察组术后6、12个月角化龈宽度为(3.27±1.14)、(3.13±0.97)mm,均明显大于对照组[(2.65±0.98)、(2.47±0.89)mm],差异均有统计学意义(P<0.05)。观察组术后PIS 0级、Ⅰ级、Ⅱ级、Ⅲ级及Ⅳ分别占1.96%、17.65%、31.57%、49.02%和0,低于对照组5.88%、21.57%、47.06%、21.57%和3.92%,差异均有统计学意义(P<0.05)。结论上颌前牙区骨缺损GBR术后采用CGF双膜覆盖,有助于减少骨吸收,增强成骨能力和口腔软组织再生能力,值得临床予以推广应用。 Objective To study the osteogenesis ability and oral tissue regeneration effect of the concentrated growth factor(CGF)double membrane covering applied to the maxillary anterior bone defect guided bone regeneration surgery.Methods A total of 102 patients with bone defects in the maxillary anterior teeth who were admitted to Handan First Hospital from January 2019 to October 2020 were prospectively selected as the research objects,and divided into two groups according to the random number table,each group 51 cases.The observation group was given with guided bone regeneration(GBR)+Bio-Oss bone meal+CGF membrane+Haiao biomembrane,and the control group was given with GBR+Bio-Oss bone meal+Haiao Biofilm.The postoperative implant horizontal and vertical bone mass and keratinized gingival width data were measured,and the Jemt papilla index score(PIS)was used to evaluate the effect of oral soft tissue regeneration.Results The horizontal and vertical bone mass of the implant in the observation group were significantly greater than those in the control group at 6 and 12 months after operation,and the bone resorption in the horizontal and vertical directions were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).The widths of keratinized gingiva in the observation group were(3.27±1.14)mm,(3.13±0.97)mm,which were significantly larger than that in the control group[(2.65±0.98)mm,(2.47±0.89)mm]at 6 months and 12 months after operation,and the differences were statistically significant(P<0.05).The postoperative PIS grades 0,Ⅰ,Ⅱ,ⅢandⅣin the observation group accounted for 1.96%,17.65%,31.57%,49.02%and 0,respectively,which were lower those that in the control group(5.88%,21.57%,47.06%,21.57%and 3.92%),the differences were statistically significant(P<0.05).Conclusion Bone defect in the maxillary anterior region is covered with CGF double membrane after GBR,which helps reduce bone resorption,enhances the ability of bone formation and oral soft tissue regeneration,and is worthy of clinical application.
