乌头汤联合塞来昔布治疗风寒湿痹证膝关节骨性关节炎的临床观察

Clinical Observation of Wutou Decoction Combined with Celecoxib in Treating Knee Osteoarthritis with Wind-Cold-Dampness Syndrome

目的观察乌头汤联合塞来昔布治疗膝关节骨性关节炎的临床疗效。方法选取2018年1月—2020年1月期间解放军第989医院收治的风寒湿痹证膝骨性关节炎患者200例,按随机数字表法分为对照组和治疗组,每组各100例。对照组给予塞来昔布治疗,治疗组在对照组治疗基础上给予乌头汤加减。采用疼痛视觉模拟评分(Visual Analogue Scale,VAS)、日本骨科协会(Japanese Orthopedic Association1,JOA)膝关节功能评价法、骨关节炎评分法(The Western Ontario and McMaster Universities Osteoarthritis Index,WOMAC)、膝骨关节炎严重程度指数(Index of severity for osteoarthritis,ISOA)对两组患者膝关节疼痛、功能及病情变化进行分析,并分析两组患者炎性因子水平变化及不良反应情况。结果治疗后治疗组总有效率为95%(95/100)高于对照组76%(76/100),差异有统计学意义(P<0.05)。两组患者治疗后VAS评分较治疗前降低,JOA评分较治疗前升高,差异有统计学意义(P<0.05);且治疗组治疗后VAS评分、JOA评分均明显优于对照组,两组比较,差异有统计学意义(P<0.05)。两组患者治疗后WOMAC评分、ISOA评分、炎性因子水平均较治疗前降低,差异有统计学意义(P<0.05),且治疗组WOMAC评分、ISOA评分、炎性因子水平均较对照组明显降低,差异有统计学意义(P<0.05)。治疗期间,两组均未出现药物相关性不良反应。结论乌头汤联合塞来昔布的中西医结合治疗可有效改善膝关节骨性关节炎患者的临床症状,提高临床治疗效果。

Objective To observe the clinical effect of Wutou Decoction combined with Celecoxib in the treatment of knee osteoarthritis.Methods A total of 200 patients with knee osteoarthritis with wind-cold-dampness arthralgia who were admitted to the 989 th Hospital of the People’s Liberation Army from January 2018 to January 2020 were selected and divided into a control group and a treatment group according to the random number table method,with 100 cases in each group.The control group was given celecoxib,and the treatment group was given Wutou decoction based on the treatment of the control group.Pain Visual Analogue Scale(VAS),Japanese Orthopedic Association1(JOA)knee joint function evaluation method,Osteoarthritis score(The Western Ontario and McMaster Universities Osteoarthritis Index,WOMAC),knee joint The index of severity for osteoarthritis(ISOA)was used to analyze the changes of knee joint pain,function and condition in the two groups of patients,and the changes in the levels of inflammatory factors and adverse reactions in the two groups of patients were also analyzed.Results After treatment,the total effective rate in the treatment group was 95%(95/100)higher than 76%(76/100)in the control group,and the difference was statistically significant(P<0.05).After treatment,the VAS scores in the two groups were lower than those before treatment,and the JOA scores were higher than those before treatment,and the difference was statistically significant(P<0.05).The difference was statistically significant(P<0.05).After treatment,the WOMAC score,ISOA score,and inflammatory factor levels in the two groups were lower than those before treatment,and the difference was statistically significant(P<0.05).decreased,and the difference was statistically significant(P<0.05).During the treatment period,there were no drug-related adverse reactionsin the two groups.Conclusion The combination of Wutou Decoction and celecoxib can effectively improve the clinical symptoms of patients with knee osteoarthritis and improve the clinical treatment effect.
