灯盏生脉胶囊联合丁苯酞注射液对轻中度脑梗死患者侧支循环、血管内皮功能的影响

Effects of Dengzhan Shengmai Capsule Combined with Butyphthalide Injection on Collateral Circulation and Vascular Endothelial Function in Patients Suffering Mild to Moderate Cerebral Infarction

目的:探讨灯盏生脉胶囊联合丁苯酞注射液对轻中度脑梗死患者侧支循环、血管内皮功能的影响。方法:将轻中度脑梗死患者120例随机分为研究组和对照组各60例,均给予抗血小板等常规治疗。对照组在常规治疗的基础上予丁苯酞注射液,研究组在对照组治疗的基础上联合灯盏生脉胶囊治疗。观察两组患者总有效率及侧支循环情况;比较两组患者治疗前后一氧化氮(nitric oxide,NO)、内皮素1(endothelin-1,ET-1)、氧化低密度脂蛋白(ox-LDL)水平。结果:研究组患者总有效率为88.33%(53/60),高于对照组的63.33%(38/60)(P<0.05);研究组患者治疗后侧支循环开放率为81.67%,高于对照组的63.33%(P<0.05);治疗后两组患者NO、内皮细胞依赖性血管舒张功能(flow-mediated vasodilation,FMD)水平均升高,研究组升高更明显(P<0.05),ET-1、ox-LDL水平均降低,研究组降低更明显(P<0.05),内皮细胞非依赖性血管舒张功能(nitroglycerin mediated dilation,NMD)水平两组治疗前后比较差异无统计学意义(P>0.05)。结论:灯盏生脉胶囊联合丁苯酞注射液可促进轻中度脑梗死患者侧支循环的建立、改善血管内皮功能,效果优于常规联合丁苯酞注射液治疗。

Objective:To explore the effects of Dengzhan Shengmai capsules combined with butyphthalide injection on collateral circulation and vascular endothelial function in patients suffering mild to moderate cerebral infarction.Methods:All 120 patients were randomized into the study group and the control group,with 60 cases in each group,and they were given conventional therapy including anti-platelet treatment.The control group accepted butyphthalide injection on the foundation of conventional therapy,and the study group took Dengzhan Shengmai capsules based on the therapy the control group accepted.To observe total effective rate and collateral circulation in both groups;to compare the levels of NO,ET-1 and ox-LDL before and after treatment between both groups.Results:Total effective rate of the study group was 88.33%(53/60),higher than 63.33%(38/60) of the control group(P<0.05);the open rate of collateral circulation of the study group was 81.67% after treatment,higher than 63.33% of the control group(P<0.05);the levels of NO and FMD of both groups increased after treatment,the increase of the study group was more notable(P<0.05),the levels of ET-1 and ox-LDL decreased,the decrease of the study group was more notable(P<0.05).The difference had no statistical meaning in NMD before and after treatment between both groups(P>0.05).Conclusion:Dengzhan Shengmai capsules combined with butyphthalide injection could promote the establishment of collateral circulation and improve vascular endothelial function,and its effects are superior to these of conventional therapy and butyphthalide injection.
