阴道炎与高危型人乳头瘤病毒感染相关性横断面研究

Cross sectional study on the correlation between vaginitis and high-risk human papillomavirus infection

目的 探讨妇科“两癌”筛查人群中阴道炎与高危型人乳头瘤病毒(HR-HPV)感染的关系。方法 选取2021年6—10月于山西省长治市襄垣县妇幼保健计划生育服务中心妇科门诊行妇科“两癌”筛查人群1972例,收集其人口学、阴道微生态检测、HPV检测和子宫颈活体组织病理学检测信息,并对结果进行分析。结果 在1972例筛查人群中,诊断为阴道炎者共918例;外阴阴道假丝酵母菌病(vulvovaginal candidiasis,VVC)88例;阴道毛滴虫病(trichomonal vaginosis,TV)50例;子宫颈病变25例,其中低级别鳞状上皮内病变(low-grade squamous intraepithelial lesion,LSIL)6例,高级别鳞状上皮内病变(high-grade squamous intraepithelial lesion,HSIL)4例,其他子宫颈炎症15例;HR-HPV阳性273例。细菌性阴道炎(bacterial vaginosis,BV)、BV中间型、需氧性阴道炎(aerobic vaginosis,AV)与HR-HPV共同感染发生风险的OR值分别为2.63(95%CI 1.70~4.09)、2.05(95%CI1.08~3.91)和5.92(95%CI 3.24~10.84),以上结果差异均有统计学意义(P<0.05)。结论 细菌性阴道炎和需氧性阴道炎与HR-HPV感染具有一定相关性。

Objective To analysis the correlation between female vaginitis with high risk human papilloma virus(HR-HPV)infection in outpatient.Methods A total of 1914 outpatient subjects in Xiangyuan Maternal and Child Care Hospital,Changzhi,Shanxi province were enrolled from June to October in 2021. All subjects have underwent virginal microecological test,HPV testing and histopathological examination guided by colposcopy for women with abnormal results. The results were collected and analyzed for the correlation between female lower genital track infection and HPV.Results In all 1972 subjects:918 subjects had vaginitis;88 subjects had VVC,50 subjects had TV;6 subjects with LSIL,4 with HISL;15 subjects had cervicitis;273 were infected with HR-HPV. The odds ratio(OR)of bacteria vaginitis(BV),severe BV,and aerobic vaginitis(AV)with HPV infection is 2.63(95%CI 1.70-4.09)、2.05(95%CI 1.08-3.91)and 5.92(95%CI 3.24-10.84),respectively;All result are statistically significant(P<0.05).Conclusion Bacterial vaginitis and aerobic vaginitis are related to HR-HPV infection.
