无创皮肤吻合器对跟骨骨折术后切口愈合的影响

目的探讨无创皮肤吻合器对跟骨骨折术后切口愈合的影响。方法选取2016年1月至2021年6月本院收治的单侧闭合性SandersⅡ~Ⅳ型跟骨骨折患者120例,按照随机数字表法分为对照组和观察组,每组各60例。两组均采取外侧“L”形入路切开复位钢板内固定术治疗,对照组采用间断垂直褥式缝合法,观察组采用无创皮肤吻合器闭合皮肤切口,围术期均予以常规消肿及抗感染治疗。比较两组的围手术期相关指标、术后切口情况和临床疗效。结果观察组的切口缝合时间、切口渗出及足部肿胀时间均短于对照组,术后拆线疼痛评分低于对照组,切口外观满意度高于对照组,差异均有统计学意义(P<0.05)。观察组的切口并发症发生率为6.67%,低于对照组20.00%,差异有统计学意义(P<0.05)。观察组Maryland足部功能评分优良率(91.67%)高于对照组(78.33%),差异有统计学意义(P<0.05)。结论无创皮肤吻合器应用于跟骨骨折术后切口的闭合,术中缝合时间短,且术后切口恢复情况及临床疗效均优于间断垂直褥式缝合。

Objective To explore the effect of non-invasive skin stapler on wound healing after calcaneal fracture surgery.Methods 120 patients with unilateral closed Sanders type II~IV calcaneal fractures treated in our hospital from January 2016 to June 2021 were randomly divided into control group and observation group,with 60 cases in each group.Both groups were treated with open reduction and plate internal fixation through the lateral"L"approach,the control group was treated with intermittent vertical mattress suture,and the observation group was treated with non-invasive skin stapler to close the skin incision.Routine anti-swelling and anti-infection treatment were given in the perioperative period.The perioperative related indexes,postoperative incision and clinical efficacy were compared between the two groups.Results The incision suture time,incision exudation and foot swelling time in the observation group were shorter than those in the control group,the postoperative suture removal pain score in the observation group was lower than that in the control group,and the incision appearance satisfaction in the observation group was higher than that in the control group,with statistical significance(P<0.05).The incidence of incision complications in the observation group was 6.67%,which was lower than 20.00%in the control group,with statistical significance(P<0.05).The excellent and good rate of Maryland foot function score in the observation group(91.67%)was higher than that in the control group(78.33%),and the difference was statistically significant(P<0.05).Conclusion The application of non-invasive skin stapler in the closure of incision after calcaneal fracture operation has advantages of shorter suture time,and the postoperative incision recovery and clinical effect are better than intermittent vertical mattress suture.
