摘要
心脏磁共振作为一种影像学技术,可帮助检查者准确了解受检者的心脏解剖、功能、心脏代谢、心肌活性、心肌灌注、室壁运动以及冠状动脉等情况,是目前临床诊断冠心病的常规方案。既往临床常规使用的1.5 T冠状动脉MRA技术普遍为三维稳态进动脉冲序列(SSFP)而存在一定的弊端,近年来随着临床技术的快速发展,3.0 T MRI逐渐得到推广,其所使用的FLASH序列有效弥补了SSFP序列的弊端,且支持自由呼吸法,相对比而言成像空间分辨力更高,适用于全心冠状动脉的诊断。可以预见冠心病临床诊断的未来发展必将是“一站式”的,即通过1次检测明确受检者的心脏形态、心肌灌注、心肌活性、心脏室壁运动等情况,从而帮助检查者在术前对受检者的冠脉临界狭窄心肌灌注、心肌存活等信息形成明确的认知,进而判断是否为受检者进行开通。由此可见,未来心脏疾病的临床诊断与治疗中,3.0 T心脏MRI必将发挥至关重要的作用。所以本次通过汇总既往研究资料,对冠心病临床诊治中3.0 T心脏MRI的应用价值进行综述,希望可以为相关人员提供有价值的参考。
Cardiac magnetic resonance, as an imaging technique, can help examiners accurately understand the subject’s cardiac anatomy, function, cardiac metabolism, myocardial activity, myocardial perfusion, ventricular wall motion, and coronary arteries. It is the conventional clinical diagnosis plan for coronary heart disease at present. The 1.5 T coronary MRA technology routinely used in clinical practice is generally a three-dimensional steady-state precession pulse sequence(SSFP) and has certain drawbacks.In recent years, with the rapid development of clinical technology, 3.0 T MRI has gradually been promoted.The FLASH sequence used in it effectively compensates for the disadvantages of the SSFP sequence, and supports free breathing, which has a higher imaging spatial resolution, and is suitable for the diagnosis of whole-heart coronary arteries. It is foreseeable that the future development of clinical diagnosis of coronary heart disease will be "one-stop", that is, to clarify the subject’s heart shape, myocardial perfusion,myocardial activity, heart wall motion, etc. through a single test, so as to help the examiner in the operation form a clear understanding of the subject’s coronary critical stenosis myocardial perfusion,myocardial survival and other information, and determine whether to open the subject. It can be seen that in the future clinical diagnosis and treatment of heart disease, 3.0 T heart MRI is bound to play a vital role.Therefore, by summarizing previous research data, the application value of 3.0 T cardiac MRI in the clinical diagnosis and treatment of coronary heart disease is reviewed, and it is hoped that it can provide valuable references for relevant personnel.
