不同术式治疗前列腺增生症的疗效比较

Comparison of different surgical methods in the treatment of benign prostatic hyperplasia

目的 比较经尿道前列腺剜除术与经尿道前列腺电切术治疗前列腺增生症的疗效及安全性。方法 选取2019年5月—2020年10月佛山市第一人民医院禅城医院收治的前列腺增生症患者80例为研究对象,根据不同手术方式将其均分为对照组和观察组各40例,对照组患者行经尿道前列腺电切术治疗,观察组患者行经尿道前列腺剜除术治疗,观察并比较两组围术期各指标、安全性及术后疗效。结果 观察组尿管留置时间、住院时间、手术时间短于对照组,前列腺切除量多于对照组,且输血量及并发症发生率均小于对照组,差异均有统计学意义(P<0.05),观察组术后最大尿流量、残尿量、前列腺症状评分、生活质量评分均优于对照组,差异均有统计学意义(P<0.05)。结论 经尿道前列腺剜除术与经尿道前列腺电切术比较,术中出血量更少,剜除腺体更彻底,术后并发症风险更低、恢复更快、住院时间更短,优势明显,具有较好应用前景。

Objective To compare the efficacy and safety of transurethral enucleation of prostate(TEP) and transurethral resection of prostate(TURP) in the treatment of benign prostatic hyperplasia(BPH). Methods From May 2019 to October 2020, 80 patients with BPH treated in Chancheng Hospital of Foshan First People’s Hospital were selected as the research object. According to different operation methods, they were divided into control group and observation group, with 40 cases in each group. Patients in the control group were treated with TURP, and patients in the observation group were treated with TEP. The perioperative indexes, safety and postoperative efficacy of the two groups were observed and compared. Results The urethra indwelling time, hospitalization time and operation time of the observation group were shorter than those of the control group, the amount of prostatectomy was more than that of the control group, and the blood transfusion and the incidence of complications were less than those of the control group, and the differences were statistically significant(P<0.05). The maximum urinary flow, residual urine volume, prostate symptom score and quality of life score of the observation group were better than those of the control group, and the differences were statistically significant(P <0.05). Conclusion Compared with TURP, TEP had less intraoperative bleeding, more thorough enucleation of glands, lower risk of postoperative complications,faster recovery, shorter hospital stay, which had obvious advantages and good application prospects.
