摘要
目的 分析CT血管造影(CTA)技术在冠状动脉成像中的应用效果,为冠心病的诊治提供帮助。方法 选取2020年6月—2021年5月于天津市胸科医院检查中检出的80例初筛疑似冠心病病例纳入研究,采用CTA技术进行临床诊断,并采用后续图像重建技术提升图像质量,通过与数字减影造影(DSA)的诊断结果相对比,通过对两种影像学技术在诊断冠心病患者冠脉狭窄以及诊断的敏感性、特异度以及准确度方面的效果进行评估,观察CTA的临床应用价值。结果 CTA诊断冠心病的敏感度为100.0%,特异度为33.3%,准确性为97.5%,与DSA相比,差异无统计学意义(P>0.05)。采取CTA进行各冠脉分支狭窄情况的探查,发现在左主干、左前降支、左回旋支、左冠主支狭窄情况的检出率与DSA相比差异无统计学意义(P>0.05)。结论 进行冠脉狭窄的判断有助于明确诊断冠心病,有助于通过对疾病的严重程度临床制订系统化治疗方案,提升预后。采取CTA进行冠脉成像诊断,能够明显提升冠脉狭窄检出率,成像质量好,且与DSA技术相比,具有微创、操作简单、伤害低等优势,还能够为后续诊疗提供科学依据。
Objective To analyze the effect of CT angiography(CTA) in coronary angiography, in order to provide help for the diagnosis and treatment for coronary heart disease. Methods A total of 80 cases of suspected coronary heart disease detected in Tianjin Chest Hospital from June 2020 to May 2021 were included in the study. CTA technology was used for clinical diagnosis, and follow-up image reconstruction technology was used to improve image quality. Compared with the diagnostic results of digital subtraction angiography(DSA), the clinical value of CTA was observed by evaluating the effects of two imaging techniques in the diagnosis of coronary stenos is and the sensitivity, specificity and accuracy of diagnosis in patients with coronary heart disease. Results The sensitivity, specificity and accuracy of CTA in the diagnosis of coronary heart disease were 100.0%, 33.3% and 97.5%. There was no significant difference compared with DSA(P>0.05). CTA was used to detect the stenosis of each coronary artery branch, and it was found that the detection rate of stenosis of left main artery, left anterior descending artery, left circumflex artery and left main coronary artery had no significant difference compared with DSA(P>0.05).Conclusion The judgment of coronary stenosis was helpful to make a clear diagnosis of coronary heart disease, make a systematic clinical treatment plan for the severity of the disease, and improve the prognosis.CTA for coronary imaging diagnosis could significantly improve the detection rate of coronary stenosis and had good imaging quality. Compared with DSA technology, CTA had the advantages of minimally invasive,simple operation and low injury. It could also provide a scientific basis for subsequent diagnosis and treatment.
