动态心电图与常规心电图诊断妊娠合并心脏病的应用

Application of dynamic electrocardiogram and conventional electrocardiogram in diagnosis of pregnancy complicated with heart disease

目的探究动态心电图与常规心电图诊断妊娠合并心脏病的应用。方法选取我院2018年12月-2020年1月收治的125例妊娠合并心脏病患者。采用随机数字表法分为实验组(n=63)和对照组(n=62),实验组进行动态心电图检查,对照组进行常规心电图检查,对比两组患者检出情况和阳性诊断率。结果相比于对照组特发性室性心律失常检出率4.83%、阵发性室上性心动过速检出率6.45%、窦性心动过速检出率8.06%、心房纤颤检出率4.83%、ST-T改变检出率6.45%,实验组特发性室性心律失常检出率20.63%、阵发性室上性心动过速检出率22.22%、窦性心动过速检出率20.63%、心房纤颤检出率20.63%、ST-T改变检出率25.39%显著更高(P<0.05);相比于对照组阳性符合率84.87%,实验组阳性符合率92.43%显著更高;实验组灵敏度为98.21,特异度为30.77,对照组灵敏度为95.28,特异度为5.26,实验组准确度为91.20,对照组准确度为81.60。结论妊娠合并心脏病患者主要以特发性室性心律失常、阵发性室上性心动过速、窦性心动过速、心房纤颤、ST-T改变为主,且动态心电图比常规心电图更适合妊娠合并心脏病的诊断。

Objective To explore the application of dynamic electrocardiogram and conventional electrocardiogram in the diagnosis of pregnancy complicated with heart disease.Methods A total of 125 cases of pregnancy complicated with heart disease admitted to our hospital from December 2018 to January 2020 were selected and randomly divide into the experimental group(n=63)and the control group(n=62).The experimental group was subjected to dynamic electrocardiogram,and the control group was subjected to routine ECG examination.The detection conditions and positive diagnosis rates of the two groups were compared.Results Compared with the control group(4.83%,6.45%,8.06%,4.83%,6.45%),the detection rates of idiopathic ventricular arrhythmia,the paroxysmal supraventricular tachycardia,the sinus tachycardia,the atrial fibrillation,and the ST-T change detection of the experimental group were significantly higher(20.63%,22.22%,20.63%,20.63%,25.39%)(P<0.05).Compared with the positive coincidence rate of 84.87%in the control group,the positive coincidence rate of 92.43%in the experimental group was significantly higher.In the experimental group,the sensitivity was 98.21,the specificity was 30.77;in the control group,the sensitivity was 95.28,the specificity was 5.26;the accuracy of the experimental group was 91.20,and the accuracy of the control group was 81.60.Conclusion Pregnancy patients with heart disease are mainly characterized by idiopathic ventricular arrhythmia,paroxysmal supraventricular tachycardia,sinus tachycardia,atrial fibrillation,ST-T changes,and the dynamic electrocardiogram is more important than the conventional electrocardiogram,which is more suitable for diagnosis of pregnancy complicated with heart disease.
