摘要
目的探讨慢加急性乙型肝炎肝衰竭前期患者血浆二胺氧化酶(DOA)与肠道菌群相关性。方法选取2019年1月至2020年2月收入我院的慢加急性乙型肝炎肝衰竭前期患者72例作观察组及同期体检的健康人64例作对照组。比较两组患者血浆DOA水平与肠道菌群门与属的相对丰度,并进行相关性分析。结果观察组血浆DOA水平高于对照组(P<0.05)。在门水平上,观察组较对照组相比厚壁菌相对丰度降低,拟杆菌及放线菌相对丰度增加;在属水平上,不明肠杆菌科、韦荣球菌、链球菌、肠球菌及乳酸菌丰度显著增加。本研究将门、属水平肠道差异菌群与血浆DOA水平进行相关分析,得到血浆DOA水平与链球菌(r=0.642,P<0.001)、韦荣球菌(r=0.412,P<0.001)相对丰度呈正相关。结论慢加急性乙型肝炎肝衰竭前期患者血浆DOA水平与病情严重程度有关,且肠道菌群构成与物种均发生改变,其中异常富集的链球菌、韦荣球菌与血浆DOA水平紧密相关。
Objective To investigate the correlation between plasma diamine oxidase(DOA)and intestinal flora in patients with chronic onset acute hepatitis B liver failure.Methods A total of 72 patients with chronic acute hepatitis B liver failure admitted to our hospital from January 2019 to February 2020 were selected as the observation group and 64 healthy people who underwent physical examination during the same period as the control group.The plasma DOA level and the relative abundance of intestinal flora and genera were compared between the two groups of patients,and their correlation was analyzed.Results The plasma DOA level of the observation group was higher than that of the control group(P<0.05).At the phylum level,compared with the control group,the observation group had a lower relative abundance of Firmicutes,while the relative abundance of Bacteroides and Actinomyces increased;at the genus level,the abundance of Enterobacteriaceae,Veillonococcus,Streptococcus,Enterococcus and lactic acid bacteria increased significantly.In the study,the correlation analysis was performed between the different intestinal flora at the level of phylum and genus and concluded that the plasma DOA level was positively correlated with the relative abundance of streptococcus(r=0.642,P<0.001)and Veillonella(r=0.412,P<0.001).Conclusion The plasma DOA level of patients with chronic acute hepatitis B liver failure in the early stage is related to the severity of the disease,and the composition and species of the intestinal flora have changed where abnormally enriched streptococcus and virion cocci are closely related to plasma DOA levels.
