囊肿塞治器自体吸引辅助治疗对牙源性颌骨囊肿患者囊肿体积的影响

The Effect of Autogenous Suction Assisted Therapy with Cyst Plug on the Cyst Volume of Atients with Odontogenic Jaw Cyst

目的探讨囊肿塞治器自体吸引辅助开窗减压术对累及≥2牙位大型牙源性颌骨囊肿患者囊肿体积的影响。方法回顾性分析2020年1月至2021年1月于本院确诊的46例累及≥2牙位大型牙源性颌骨囊肿患者,根据有无采用囊肿塞治器自体吸引辅助将患者分为观察组24例和对照组22例。对照组患者采用颌骨开窗减压术治疗基础上加用囊治塞治疗,观察组采用颌骨开窗减压术加用囊肿塞治器自体吸引治疗。分别于治疗前后,采用双能X线检测术后骨质的密度。采用mimics15.0系统分别测量术前及术后6、12、18个月的囊腔体积及表面积。观察记录两组治疗过程中手术时间、术中出血量。结果观察组患者术后囊腔密度明显高于对照组(P<0.05);观察组患者术后6、12、18个月囊肿缩小体积幅度明显低于对照组(P<0.05);观察组患者术后6、12、18个月囊肿缩小面积幅度明显低于对照组(P<0.05)。结论囊肿塞自体吸引辅助开窗对累及≥2牙位大型牙源性颌骨囊肿的患者,可加速囊肿体积及面积的缩小,缩短治疗疗程,促进囊肿周围骨的重建。

Objective To investigate the effect of cystic occlusion device self-aspiration assisted fenestration decompression on the cyst volume of patients with large odontogenic jaw cysts involving≥2 teeth.Methods A retrospective analysis of 46 patients with large odontogenic jaw cysts involving≥2 teeth diagnosed in our hospital from January 2020 to January 2021 were retrospectively analyzed.The patients were divided into observations according to whether they were assisted by auto-aspiration with cyst plugs Group(n=24)and control group(n=22).The patients in the control group were treated with fenestration decompression of the jaws plus capsule treatment,and the observation group was treated with fenestration decompression of the jaws plus autogenous aspiration treatment with a cyst tamponade.Before and after treatment,dual-energy X-rays were used to detect postoperative bone density.The mimics15.0 system was used to measure the volume and surface area of the cyst before operation and at 6,12,and 18 months after operation.Observe and record the operation time and intraoperative blood loss during the treatment of the two groups.Results The postoperative cyst density of the observation group was significantly higher than that of the control group(P<0.05);the size of cyst reduction in the observation group was significantly lower than that of the control group at 6,12,and 18 months after surgery(P<0.05);the observation group was The area of cyst reduction at 6,12,and 18 months after surgery was significantly lower than that of the control group(P<0.05).Conclusion The self-aspiration assisted fenestration of cyst plugs can accelerate the reduction of the volume and area of the cyst,shorten the treatment course,and promote the reconstruction of the bone around the cyst for patients with large odontogenic jaw cysts involving≥2 teeth.
