肺癌^(18)F-FDG PET/CT代谢参数与EGFR基因突变的相关性分析 被引量:1

Correlation Between^(18)F-FDG PET/CT Metabolic Parameters and EGFR Gene Mutation in Lung Cancer

摘要 目的探究^(18)F-FDG PET/CT代谢参数与肺腺癌表皮生长因子受体(EGFR)突变的相关性,并评估其预测突变的效能。方法回顾性分析2019年1月至2021年7月于内蒙古自治区人民医院行PET/CT检查的患者,筛选出73例确诊肺腺癌的患者,收集临床资料、EGFR基因检测结果。用统计学方法分析性别、年龄、吸烟史、疾病分期以及PET/CT相关代谢参数与EGFR突变之间的关系。结果73例患者中EGFR突变患者33例(45.2%),女性较男性更易出现EGFR突变(χ^(2)=6.154,P<0.05;不吸烟组的EGFR突变比例较吸烟组高(χ^(2)=5.400,P<0.05;年龄、疾病分期与EGFR突变无关。EGFR突变组和野生组原发病灶的SUVmax分别为(12.22±4.87)和(12.15±4.47)(P=0.952);SUVmean为(7.39±3.06)和(7.22±2.60)(P=0.791);MTV为10.34(5.07,19.67)cm^(3)和8.24(4.36,20.99)cm^(3)(P=0.650);TLG为69.18(37.49,144.26)和60.56(23.35,161.17)(P=0.542)。结论EGFR突变组的^(18)F-FDG PET/CT代谢参数的均值或中位数稍高,但差别均无统计学意义。性别、吸烟史等临床特征与EGFR突变存在相关性。 Objective To explore the correlation between ^(18)F-FDG PET/CT metabolic parameters and epidermal growth factor receptor(EGFR)mutations in lung adenocarcinoma and to assess their efficacy in predicting mutations.Methods Patients who underwent PET/CT at Inner Mongolia Autonomous Region People’s Hospital from January 2019 to July 2021 were retrospectively analyzed,and 73 patients with confirmed lung adenocarcinoma were screened to collect clinical data and EGFR gene test results.The relationship between gender,age,smoking history,disease stage,and PET/CT-related metabolic parameters and EGFR mutations was analyzed statistically.Results Of the 73 patients,33 patients had EGFR mutations(45.2%),and women were more likely to develop EGFR mutations than men(χ^(2)=6.154,P<0.05).The proportion of EGFR mutations was higher in the non-smoking group compared in the smoking group(χ^(2)=5.400,P<0.05).And age,disease stage were not associated with the EGFR mutation.The SUVmax of the primary lesions in the EGFR mutant group and the wild group were(12.22±4.87)and(12.15±4.47),respectively(P=0.952);The SUVmean were(7.39±3.06)and(7.22±2.60),respectively(P=0.791);The MTV were 10.34(5.07,19.67)cm^(3) and 8.24(4.36,20.99)cm^(3),respectively(P=0.650);The TLG were 69.18(37.49,144.26)and 60.56(23.35,161.17),respectively(P=0.542).Conclusion The mean or median of ^(18)F-FDG PET/CT metabolic parameters was slightly higher in the EGFR mutation group,but none of the differences were statistically significant.Clinical characteristics such as gender and smoking history were correlated with EGFR mutations.
