控制性减压术对中老年重型颅脑损伤的疗效观察 被引量:2

摘要 目的:探讨采用控制性减压术治疗中老年重型颅脑损伤患者的效果。方法:选择中老年重型颅脑损伤患者(70例)依据治疗方法不同分为2组,行大骨瓣减压术的35例作为对照组,行控制性减压术的35例作为观察组,比较2组临床疗效及并发症情况。结果:观察组治疗总有效率高于对照组(97.14%vs 71.43%,P<0.05);观察组术后3个月不良反应发生率低于对照组(5.71%vs 28.57%,P<0.05),格拉斯哥预后评分(GOS)[(4.36±0.84)vs(3.25±0.63)分]、神经行为认知状况测试(NCSE)评分[(66.24±8.55)vs(50.87±6.18)分]高于对照组,差异有统计学意义(P<0.05)。结论:采用控制性减压术治疗中老年重型颅脑损伤患者,临床效果好,不良反应少且症状改善明显。 Objective:To evaluate the effect of using controlled decompression in the treatment of middle-aged and elderly patients with severe craniocerebral injury.Methods:The middle-aged and elderly patients with severe craniocerebral injury(70 cases)were divided into two groups according to the time of selection.35 cases treated with decompressive craniectomy,and cases treated with large trauma craniotomy were used as the control group,and 35 cases treated with controlled decompression were used as the observation group.The clinical efficacy and complications of the two groups were compared.Results:The overall effective rate of the observation group(97.14%)was higher than that of the control group(71.43%)(P<0.05).The incidence of adverse reactions at 3 months after surgery in the observation group(5.71%)was lower than that in the control group(28.57%)(P<0.05).The Glasgow prognosis score(GOS)(4.36±0.84)and neurobehavioral cognitive status test(NCSE)(66.24±8.55)in the observation group were higher than those in the control group(3.25±0.63)and(50.87±6.18)and the difference was statistically significant(P<0.05).Conclusion:For middle-aged and elderly patients with severe craniocerebral injury,controlled decompression has good effects,fewer adverse reactions and more obvious improvement in symptoms.
