小剂量茶碱缓释片联合沙美特罗替卡松粉吸入剂治疗重度稳定期COPD效果分析

Effect of low-dose theophylline sustained-release tablets combined with inhaler salmeterol ticasone powder in the treatment of severe stable COPD

目的研究小剂量茶碱缓释片联合沙美特罗替卡松粉吸入剂治疗重度稳定期慢性阻塞性肺疾病(Chronic Obstructive Pulmonary Disease,COPD)的效果。方法选取2019年7月~2020年7月我院重度稳定期COPD患者114例,按照治疗方案分为对照组、观察组。对照组49例,采用沙美特罗替卡松粉吸入剂治疗,观察组65例,给予沙美特罗替卡松粉吸入剂联合小剂量茶碱缓释片治疗。比较两组疗效及治疗前、治疗6个月后慢性阻塞性肺疾病评估测试量表(COPD Assessment Test,CAT)、6min步行距离(6minute walking distance,6MWD)、第1s用力呼气容积(forced expiratory volume in one second,FEV_(1))、用力肺活量(forced vital capacity,FVC)、深吸气量(inspiratory capacity,IC)、呼吸困难程度及不良反应。结果观察组总有效率93.85%,对照组79.59%,二者比较差异有统计学意义(P<0.05);治疗6个月后观察组CAT评分低于对照组,6MWD、FEV_(1)、FVC、IC高于对照组,呼吸困难程度优于对照组,差异均有统计学意义(P<0.05);两组不良反应发生率比较差异无统计学意义(P>0.05)。结论小剂量茶碱缓释片联合沙美特罗替卡松粉吸入剂治疗重度稳定期COPD疗效显著,能缓解病情,提高6MWD与肺功能,减轻呼吸困难程度,安全性高。

Objective To study the effect of low dose theophylline sustained-release tablets combined with inhalant salmeterol ticasone powder in the treatment of severe stable chronic obstructive pulmonary disease(COPD).Methods A total of 114 patients with severe stable COPD in our hospital from July 2019 to July 2020 were retrospectively selected and divided into control group(n=49)and observation group(n=65)according to the wishes of patients and their families.The control group was treated with salmeterol ticasone powder inhaler,and the observation group was treated with low dose of theophylline sustained release tablets on the basis of control group.Efficacy,COPD Assessment Test(CAT),6 minute walking distance(6MWD),forced expiratory volume in one second(FEV_(1)),forced vital capacity(FVC),Inspiratory Capacity(IC),degree of dyspnea and adverse reactions before and 6 months after treatment were compared between two groups.Results The total effective rate of observation group(93.85%)was higher than that of control group(79.59%),the differences were statistically significant(P<0.05).After 6 months of treatment,CAT score of observation group was lower than that of control group,6MWD,FEV 1,FVC and IC were higher than those of control group,and dyspnea degree was better than that of control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Small dose of theophylline sustained release tablets combined with inhalation of salmeterol ticasone powder in the treatment of severe stable COPD has a significant effect,can alleviate the disease,improve 6MWD and lung function,reduce the degree of dyspnea,high safety.
