摘要
目的探讨猪肺磷脂注射液联合无创通气在新生儿呼吸窘迫综合征(NRDS)中的疗效。方法选取2019年9月-2021年9月天津医科大学总医院收治的82例NRDS患儿,依照随机数字表法将其分为观察组与对照组,每组41例。对照组应用无创双水平正压通气(BiPAP)治疗,观察组在对照组基础上联合猪肺磷脂注射液治疗,比较两组临床疗效、治疗相关指标(呼吸困难改善时间、无创通气时间、转有创通气率)、血氧指标[动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)、氧合指数(OI)]及并发症情况。结果观察组治疗有效率为97.56%,高于对照组的85.37%(P<0.05);观察组呼吸困难改善时间、无创通气时间短于对照组,且转有创通气率低于对照组(P<0.05);观察组PaO2、OI高于对照组,PaCO2低于对照组(P<0.05);观察组并发症发生率为7.32%,低于对照组的24.39%(P<0.05)。结论猪肺磷脂注射液联合无创通气治疗NRDS疗效肯定,可缩短治疗时间,改善患儿血氧状态,减少并发症及有创通气中转情况。
Objective To investigate the curative effect of porcine pulmonary phospholipid injection combined with noninvasive ventilation in neonatal respiratory distress syndrome(NRDS).Methods A total of 82 children with NRDS admitted to the General Hospital of Tianjin Medical University from September 2019 to September 2021 were divided into a control group(41 cases)and an observation group(41 cases)according to the random number table.The control group was treated with noninvasive bi-level positive pressure ventilation(BiPAP),and the observation group was treated with porcine pulmonary phospholipid injection on the basis of the control group.The clinical efficacy,treatment related indicators(dyspnea improvement time,noninvasive ventilation time,invasive ventilation rate),blood oxygen index[arterial partial pressure of oxygen(PaO2),arterial partial pressure of carbon dioxide(PaCO2),oxygenation index(OI)]and complications were compared between the two groups.Results The effective rate of the observation group was 97.56%,which was higher than 85.37%of the control group(P<0.05).The improvement time of dyspnea and noninvasive ventilation time in the observation group were shorter than those in the control group,and the invasive ventilation rate was lower than that in the control group(P<0.05).PaO2 and OI in the observation group were higher than those in the control group,PaCO2 was lower than that in the control group(P<0.05).The incidence of complications in the observation group was 7.32%,which was lower than 24.39%in the control group(P<0.05).Conclusion Porcine pulmonary phospholipid injection combined with noninvasive ventilation in the treatment of NRDS is effective,which can shorten the treatment time,improve the blood oxygen status of children,reduce complications and invasive ventilation transit.
